Objective : To investigate the drug resistance Observation and drug-related factors of The long Lamivudine treatment for activity decompensated cirrhosis from chronic hepatitis B.Methods: 77 cases of patients with decompensated liver cirrhosis were retrospectively analyzed between January 2000 and October 2010 in our hospital. The clinical observation of drug resistance, and resistance to treatment Child-pugh classification, HBeAg positive and HBeAg negative, HBV DNA load, serunm total bilirubin (TB), alanine aminotransferase (ALT); the HBeAg-negative and seroconversion in the treatment, HBV DNA negative and clearance time, the reduction rate of HBV DNA.Results: The drug resistance percentage Of the long Lamivudine treatment for activity decompensated cirrhosis from chronic hepatitis from the first year to the frist ten years was 14.5%,25.8%,33.87%,43.55%,50%,56.45%,61.29%,64.52%,67.74%and69.35%. There was statistical difference between drug resistance and the treatment of the Child-pugh classification,HBeAg positive and negative, HBV DNA load, the HBV DNA-negative and negative time ,the reduction rate of HBV DNA after 24 weeks treatment ; there was no statistical significance between drug resistance and treatment of serum total bilirubin level, alanine aminotransferase level, the occurred HBeAg seroconversion or HBeAg seroconversion, the reduction rate of HBV DNA after 12 weeks treatment. Conclusion: The drug resistance percentage Of the long-range Lamivudine treatment for activity decompensated cirrhosis from chronic hepatitis from the first year to the frist ten years was 14.5%, 25.8%, 33.87%, 43.55%, 50%, 56.45%, 61.29%, 64.52%, 67.74%, 69.35%, increase with time, the percentage increase in resistance, but after the seventh year of the progression become slow, resistance close to the stable. Lamivudine in patients with low resistance may be related to treatment with high Child-pugh classification, HBeAg-positive,low HBV DNA load, early HBV DNA negative, the large reduction rate of HBV DNA after 24 weeks treatment; and may be no significant correlation to the height of bilirubin level, alanine aminotransferase level ,the HBV DNA-negative and the reduction rate of HBV DNA after 12 weeks treatment。...
