Objective 1.To assess strain changes after percutaneous coronary intervention early in patients with acute myocardial infarction by left ventricular long-axis two-dimensiondal speckle tracking imaging in different directions.2. To evaluate the short-term effect of emergency percutaneous coronary intervention on patients with acute myocardial infarction.Methods Using STI, the parameters were repeatedly measured in forty-eight patients with acute myocardial infarction after percutaneous coronary intervention,in 3rd day and first month. Left ventricular long-axis peak systolic longitudinal strain were measured.Thirty-six normal subjects were enrolled as control group. Analyze the data with statistics method.Results 1. Of all patients, 1-vessel disease was present in 27(56.25%), 2-vessel disease in 16(33.33%), and 3-vessel disease in 5 patients(10.41%). There are 60 infarction related artery totally, in which 4 left main artery(6.67%),29 anterior desending branch(48.33%),11 circumflex branch(18.33%),16 right coronary artery(26.67%). All had successful operation.2. Compared with the control group, PCI patients was significantly lower systolic longitudinal strain. The curve flat,or even reverse, curve disorganized, and peak time delays.3. There days after PCI, PCI group left ventricular myocardial systolic peak strain were lower than those in control group, especially before the wall basal segments, middle segment,sidewall intermediate, inferior basal segments, intermediate, formerinterval intermediate, after the interval of the minddle section, apex level significantly lower(P <0.01). After 1 month follow-up to the left ventricular myocardium long axis of the same segment of the peak systolic longitudinal strain improved significantly, but the middle section of the anterior wall, lateral middle section, posterior middle segment, middle segment of inferior wall, the middle segment of anterior septum and apical segment is still reduced significantly (P <0.05).4. To follow 1 month,PCI group left ventricular myocardial systolic peak strain was improved in part segment significantly (P <0.01 or P <0.05).5. From 3rd day operation to follow up of 1 month, PCI patients Left ventricular ejection fraction was improved,but were lower than those in control group significantly(P <0.05).Conclution 1. the longitudinal strain curve flat, or even reverse, curve disorder, the peak time delay varies in patients acute myocardial infarction early after PCI.2. Regional systolic function damaged because of myocardial ischemia in patients with acute myocardial infarction. left ventricular myocardial longitudinal systolic peak strain and left ventricular ejection fraction were degraded.3. PCI can relieving infarction related artery,recover global and regional systolic function. Then longitudinal strain and left ventricular ejection increased. 4. Regional wall motion of left ventricule can be evaluated accurately and sensitively with Speck Tracking Imaging.
