Objective GDM(Gestational Diabetes Mellitus,GDM)is a frequently disease in pregnant woman, It's take a lot of complications, threatend the health of pregnant woman and the fetal. IR(Insulin Resistance,IR) result to GDM and T2DM(Type 2 Diabetes Mellitus, T2DM). The former research indicated that the activated TLR4(Toll like receptor 4,TLR4) and NF-кB(Nuclear factor kappa B, NF-кB) result to IR. If Activated the pass way of LPS-TLR4-Nf-κB , could release a lot of inflammatory factor, the pass way of LPS-TLR4-Nf-κB is a classic pass way. To investigate the expression of LPS-TLR4-Nf-κB pathway in peripheral blood mononuclear cells of gestational diabetes, normal pregnant women and women of childbearing age and explore the pathogenesis of GDM.Methods We chose 20 healthy pregnant women, 20 normal pregnant women as control group and 20 gestational diabetes mellitus patient as GDM group. We Collected blood(5ml) of each patients, isolated mononuclear cells, stimulated and nurtured with LPS, detected TLR4 and Nf-κB mRNA expression of mononuclear cells with RT-PCR.Determined the level of IL-1,IL-10 and TNF-a in the supernatant of medium with ELISA.Results There are differences between the expression of TLR4 mRNA and Nf-κB mRNA in GDM group and control group,the differences are clearly statistically significant(p=0.000). And also correlations between the GDM and normal pregnant women group (rGDM=0.768,rnormal pregnant women=0.833), the correlations are clearly statistically significant(p<0.05).The correlations also in women of childbearing age (r=0.719). IL-1, IL-10 and TNF-αexpression levels were 66.15±1.66,43.32±0.87 and 4.71±0.27 of GDM group, IL-1, IL-10 and TNF-αexpression levels were 14.88±0.80,12.49±0.28 and 0.89±0.39 of Normal pregnant women group, IL-1, IL-10 and TNF-αexpression levels were of 3.58±0.22,P25=0.90;P75=1.79 and 0.26±0.02 Women of childbearing age group,Conclusion GDM patients with peripheral blood mononuclear cells in vitro stimulated by LPS and cultured in the culture medium determined by high expression of inflammatory factors. TLR4, Nf-κB mRNA abundantly expressed, suggesting LPS-TLR4-Nf-κB pathway mediates a large number of inflammatory cytokine release, in vitro and to verify the existence of the pathway in GDM patients, and inflammation mediated by Cytokine release, causing the body's inflammatory and anti-inflammatory imbalance in the incidence of GDM, from the perspective of immune and inflammatory pathogenesis of GDM provides a new theoretical support.
