Objective To observe the changes of sera IL-8 and TNF-αin infants with HSP and to evaluate the influence of methylprednisolone on them. Methods 30 children with HSP and HSPN,admitted in the first Affiliated Hospital of Anhui Medical University from October 2009 to March 2010 may were enrolled in this study,Including 18 males and 12 females; age of 5 ~ 16 (11.3±2.77) years.The diagnosis were established in the light of diagnostic criteria of HSP modified by the European League Against Rheumatism/Paediatric Rheumatology European Society in 2005. One severe HSP (non-renal involvement and severe gastrointestinal symptoms, have vomiting and blood in the stool) in 5 cases, depending on whether there is massive proteinuria (urine protein characterization≥+ + +, 24h urinary protein≥50mg/kg) in turn HSPN divided into the nephritis group (10 cases) and nephropathy group (15 cases). Selected 25 patients to the hospital as normal healthy control group of children, including 12 males and 13 females; age of 3 ~ 14 (9.5±2.37) years. Two groups of gender, age difference was not statistically significant. All patients after admission did not receive any treatment before the fasting blood collected sterile 1ml, EDTA anticoagulation, 3 000r/min centrifuge 10min, separated supernatant, -80°C frozen. MP after a course of treatment in children with blood collected on day 4 1 ml, ibid isolated supernatant. At the same time in the same way groups of healthy children sterile blood collected 1ml, separated supernatant. Double antibody sandwich ELISA method for IL-8 and TNF-αdetection. Results: Before treatment of severe HSP group, nephritis group and the nephropathy group compared with the control group and three groups before and after treatment, IL-8 and TNF-αlevel of statistical significance (P <0.01); treatment of severe HSP Serum IL-8, TNF-αlevels compared with normal control group no significant difference (P> 0.01); nephritis serum IL-8, TNF-αlevels compared with normal control group was significantly (P <0.01); nephropathy, IL-8 levels compared with normal control group was significantly (P <0.01), but levels of TNF-αcompared with the control group no significant difference (P> 0.01) . Conclusion IL-8, TNF-αare involved in the basic pathogenesis of HSP and HSPN process, and have an impact on prognosis; high-dose MP treatment of HSP, HSPN rapid onset of efficacy, safety, especially early use can effectively reduce the inflammatory response.
