Objective:Endometrial carcinoma is one of the three malignancy of the female genital tract. At present the morbidity has been rising up in the world. Cyclooxygenase-2(COX-2) overexpression in various tumors illustrates that it may participate in the tumorigenesis. It completes signal transduction through its downstream products combining to the corresponding receptors in the tumor cell, and the important one of the receptors is peroxisome proliferator-activated receptor-γ(PPAR-γ). Recently, research has found that PPAR-γcould induce tumor cell differentiation as the intersection of various signal transduction pathways. In addition, PPAR-γhas a close relationship with obesity, fat metabolism and insulin resistance, suggesting that it participate in the occurrence and development of endometrial carcinoma. In conclusion, we assumed that the COX-2mRNA and PPAR-γmRNA might be overexpression in endometrial carcinoma. We detected COX-2mRNA and PPAR-γmRNA expression in the carcinoma endometrium and normal endometrial tissue and the correlation through the RT - PCR method. Furtherly , we analyzed the relationship between the expressions of COX-2mRNA and PPAR-γmRNA and clinicopathologic characteristics, including endometrial carcinoma types, the clinical stages, histological grade and et al. We were devoted to studying the pathogenesis of endometrial carcinoma. So we can learn more about the critical pathway for the pathogenesis of endometrial carcinoma. It was hoped to find a new method to diagnose endometrial carcinoma and to guide the treatment in clinic.Methods: This study included 28 cases of endometrial carcinoma in the department of Gynecology in the Second Hospital and the Fourth Hospital of Hebei Medical University from September in 2009 to December in 2010. 20 cases of normal endometrium were obtained from patients who had total hysterectomy for the reason of uterus benign tumor. All the patients had been diagnosed by pathology. The surgery - pathology stage of endometrial cancer was done according to the 2009 International Federation of Gynecology and Obstetrics classification. No patients had history of radiotherapy, chemical medicine and hormone drug treatments. With 20 minutes after operation of total hysterectomy, human endometrium tissues including the normal endometrium and carcinoma endometrium were obtained. All of these tissues were rinsed sufficiently by DEPC water and immediately frozen in liquid nitrogen. These tissues were stored at -80℃until further detection. Reverse transcription polymerase chain reaction (RT-PCR) was used to detect the expression of COX-2mRNA, PPAR-γmRNA in normal endometrium and endometrial carcinoma. Statistical analysis of the data was performed using the Statistical Product and Service Solutions 13.0(SPSS 13.0). All the measured value were expressed as mean±standard deviation(x|ˉ±s ). The data of each factor meet normal distribution. The method of two–sample t–test, separate variance estimation t–test and ANOVA was adopted to analyze the means of COX-2mRNA, PPAR-γmRNA expression and the relationship with the histological grade, FIGO stages, myometrial invasion, histotype , lymph node metastasis. The correlation coefficient(r) between COX-2mRNA and PPAR-γmRNA was determined by linear correlation analysis. A value of P<0.05 was considered statistically significant.Results:1 The expressions of COX-2mRNA, PPAR-γmRNA were detected both in normal endometrium and endometrial carcinoma. The expression of COX-2mRNA, PPAR-γmRNA in endometrial carcinoma group was significantly higher than that in normal endometrium group(P<0.05).2 A positive correlation was found between the expression levels of COX-2mRNA, PPAR-γmRNA and histological grade(P < 0.05). The expression level of COX-2mRNA in the tissues of myometrial invasion more than half was significantly higher than those of myometrial invasion equal or less than half(P<0.05). The expression levels of COX-2mRNA, PPAR-γmRNA were positively correlate with FIGO stage(P<0.05). The expression levels of COX-2mRNA, PPAR-γmRNA did not correlate with histotype, and lymph node metastasis(P>0.05).3 In endometrial carcinoma, the expressions of COX-2mRNA and PPAR-γmRNA were positively correlated(r=0.624, P<0.05).Conclusion:1 The expressions of COX-2mRNA and PPAR-γmRNA were significantly higher in endometrial carcinoma than those in normal endometrium. And the expressions of COX-2mRNA and PPAR-γmRNA were positively correlated. It illuminated that the COX-2 and PPAR-γhad taken synergistic roles in the process and development of endometrial carcinoma.2 There exists the close relationship between the expressions of COX-2mRNA , PPAR-γmRNA and prognosis of endometrial cancer.There will have worse concequences in endometrial cancer with higher expressions of COX-2mRNA , PPAR-γmRNA. It was useful to forecast development of endometrial carcinoma by combining to detect the expressions of COX-2 and PPAR-γ.
