| Objective: This study aimed to investigate the trimetazidine load 60mg had been oral administration two hours later, the level of nitric oxide (NO) and the change of endothelium-dependent vasodilation (FMD) of brachial artery to explore trimetazidine load on endothelial function of coronary heart disease combined with hypertension, type 2 diabetes and hyperlipidemia, and the difference in three groups .Methods:1 Object:According to the general international standards, the stenosis of coronary diameter≥50% was defined as CHD. <50% was identified as NCHD.Coronary angiography was performed, and all 90 patients in the study were diagnosed as coronary heart disease.A group including 30 cases which combined with primary hypertension excluding type 2 diabetes and hyperlipidemia (male 24, female 6, age 52-77, mean age 63.30±8.46, mean height 166.70±4.50cm, mean weight 75.40±8.78kg).Thirty cases in B group combined with type 2 diabetes which complied with the standards announced by World Health Organization in 1999,excluding hypertension and hyperlipidemia, male 21 and female 9 including in the group, (ages 41- 76, mean age 60.20±8.68, mean height 167.80±4.85cm, mean weight 77.40±6.36kg).C group contained 30 cases which combined hyperlipidemia (total cholesterol more than 5.72mmol/L and (or) triglycerides more than 1.7mmol/L) excluding hypertension and type 2 diabetes, including male 22 and female 8 cases (ages 45 to 74 years, mean age 59.40±8.60, mean height 166.20±3.24cm, mean weight 73.25±7.84kg.)2 Methods: Venous blood sampling 2ml from the elbow venous was carried out when the patients in stable condition after hospitalized, NO lever was measured, and then brachial arteries were examed by color Doppler sonography, the parameters of brachial artery basic diameter values D0, and diameter values D1 after added pressure to the brachial artery were obtained. The endothelium-dependent vasodilation (FMD1) of brachial artery was calculated.Then oral load trimetazidine 60mg and bed rest for 2 hours, repeated above process, finally, FMD2 was calculated.3 Statistics: The SPSS 13.0 was used to statistical computations. Major statistical indicators are normality and homogeneity of variance. Measurement data as mean±standard deviation ( x|ˉ±s), the parameters before and after treatment were test with paired t test, compared among groups by analysis of variance. P values <0.05 were considered significant.Results:1 Trimetazidine Load 60mg had been oral administration two hours later, the plasma NO levels were statistically significant higher than before treatment in A, B and C group (A group: 47.63±4.03 vs 46.48±4.29umol/L, P<0.05; B group: 46.98±3.30 vs 45.90±3.34umol/L, P<0.05; C group: 47.42±4.17 vs 46.06±4.22umol/L, P<0.05). A, B and C group compared each other before and after taking trimetazidine load, no statistically significant (P>0.05)was found in NO levels.2 Load trimetazidine 60mg was taken two hours later, FMD increased than before treatment in A, B and C group (A group: 12.91±4.96 vs 11.53±4.82%, P<0.05; B group: 10.93±6.70 vs 9.61±6.80%, P<0.05; C group: 11.34±4.97 vs 9.97±4.96%, P<0.05). A, B and C group compared each other before and after be taken load trimetazidine. No statistically significant (P>0.05) was got in FMD.Conclusion:1 Load trimetazidine 60mg had been oral administration two hours later in coronary heart disease combined with hypertension, type 2 diabetes and hyperlipidemia, the plasma NO levels were statistically significant higher than before treatment. The effects of three groups were not statistically significant.
