Aims:Studies chronic hepatitis B patient circumference blood NK cell's frequency, the activation as well as the function characteristic, with blood serum transaminase level, viral load capacity relevance; Meanwhile according to the liver different inflammation pathology graduation, the analysis circumference blood and the liver organizes the NK cell's frequency and the activation function characteristic, has further discussed the chronic hepatitis B patient NK cell's immunology characteristic and lies between leads the liver immunologic mjury the function mechanism.Methods:In this study, 34 cases were observed in patients with HBeAg positive chronic hepatitis B, 36 patients with chronic HBV carriers and 10 healthy controls the frequency of peripheral blood NK cells and activity, of which 24 cases of HBeAg-positive chronic hepatitis B patients underwent liver biopsy. Detection of liver biochemistry and viral indicators (serum alanine aminotransferase levels and HBV load), flow cytometry of peripheral blood NK cell frequency, the expression of CD69 receptor activation; NK cell secretion of cytokines IFN-γand TNF-αand cytotoxic factors perforin and CD107 levels. Immunohistochemical detection of liver tissue immune cells (CD3+ T cells and NK cells), and cytokine expression of interferon-γand CD107a.Results:1. With chronic HBV carriers and healthy contrast, HBeAg-positive chronic hepatitis B patients was significantly reduced frequency of NK cells, but the expression of early activation receptor CD69 was significantly higher than the other two groups, serum ALT levels and positive correlation (r = 0.402, p = 0.018), suggest the activation of mature NK cells may be involved in the immune liver injury. Prompt detection of NK cell function in vitro in PMA / ionomycin or IL-12 stimulation of NK cells in patients with chronic hepatitis B more IFN-γand TNF-α. NK cells in patients with chronic hepatitis B and perforin expression than that of chronic HBV carriers CD107a and CD107a expression and serum ALT levels were positively correlated.2. According to liver inflammation grade, respectively, inflammation grade G1-2 and G3-4 between the two groups; showed inflammation of the liver in patients with grade G3-4 in peripheral blood NK cell frequency decreased, but increased the expression of CD69. Prompt detection of NK cell function in vitro in PMA / ionomycin or IL-12 stimulation, inflammation of the liver in patients with grade G3-4 expression in peripheral blood NK cell IFN-γand TNF-αincreased, and the expression of perforin and CD107a also higher than the grade of liver inflammation in patients with G1-2.3. According to liver inflammation grade, respectively, inflammation grade G1-2 and G3-4 between the two groups; found that inflammation of the liver in patients with grade G3-4 in liver tissue portal area and hepatic lobule area more than the number of CD57+ NK cells in inflammation in patients with grade G1-2, inflammation in patients with grade G3-4 expression of IFN-γwere significantly higher than the inflammation grade G1-2 patients, the degree of inflammation in patients with different CD107a expression between the liver was no significant difference.Conclusions:Patients with chronic hepatitis B cells in peripheral blood NK frequency and activated receptor expression, and cytokine secretion of growth and increased expression of cytotoxic factors and liver NK cell accumulation and inflammation of the liver injury are closely related.
