Evaluate To Clinical Pathway Using In The Treatment And Diagnose In Ulcerative Colitis Patients

ObjectivesUlcerative colitis (UC) is a kind of Inflammatory bowel disease. UC is difficult to diagnose and therapy. Using the clinical pathway to diagnose and therapy UC Patients compare to the old pathway. To evaluate the clinical pathway have advantages in efficacy, hospital costs and length of stay.Methods1. Collecting the UC patients in Guangdong hospital of TCM, Beijing hospital of TCM, Guangzhou hospital of TCM, Longhua hospital, Jiangsu hospital of TCM, Yunnan hospital of TCM, Jiangxi hospital of TCM, Fujian secondary People's Hospital diagnose and therapy according to the UC clinical pathway.2. Evaluate the clinical pathway in efficacy, hospital costs and length of stay and build the database.3. Evaluate the old pathway in efficacy, hospital costs and length of stay and build the database.4. Compare clinical pathway to old pathway and analyze the efficacy, hospital costs and length of stay.ResultUC pathway is useless to shorten the length of stay (P=0.90>0.05) If existing other reason affect on the length of stay, we could make the further study. But UC pathway can reduce the hospital costs (P<0.05).It remind us the pathway standard on the treatment and examination, so the pathway reduced the cost.and the treatment efficacy is 81.25%.Because of some data has been lost, we could not analysis the difference between using the pathway and not.To increasing the patient's Satisfaction, the highest item is the 4,14 and 15.The worst item is 11,13 and 18. According to the patient's Satisfaction, it reminds us the aspect about communication between the staff and patient could get affirmation. In the other hand, it told us the patients think they are not satisfied about our efficiency. We can improve on arranging inspection in order to increase the patient's Satisfaction.we should be try to improve on cooperating to the other department.To our department about the patient's Satisfaction, almost all patients chose the 4 point, unless 4,17 and 18 item. The result told us almost all the patients is satisfied in our hospital. But it also mean we were not perfect.The satisfaction of health care remind us that they think writing the clinical pathway would increase the workload. And the health care can foresee the clinical pathway useless to reduce the length of stay. The data also tell us the pathway was useless on improving the communication between the staff and patients.But all the health care think this pathway show the advantage of TCM. Though the research, all the staff were highly praise the management on the pathway. There is no diffence between the doctor and nurse (P>0.05)About the variation, there are 137cases existing variation(82.04%). The highest frequency is D1 (259), C4(169). Influence most on the length of stay were A6,C1,C12,C4,C5,C7 and D1. Among them, the A6 and C1 can shorten the length of stay.Though analycing on the variation, we knew the situation of patients was changing anytime. Later modifying the pathway, if we can consider on the variation, we could make the pathway useful. ConclusionUC pathway is useless to shorten the length of stay (P=0.90>0.05) UC pathway can reduce the hospital costs (P<0.05) and the treatment efficacy is 81.25%.
