Objective:Qualitative analysis and quantitative measurement of mechanical parameters were perfermed, using VVI technology on different types of carotid atherosclerotic plaques. The stabilities of different types of plaques inside the blood vessel were assessed by comparing their forced status, providing a new method to check and identify early danger plaques, showing evidence for CTM clincial diagnosis:the plaques of phlegm and blood stasis syndrome are more unstable than blood stasis.Methods:1,60 patients were selected.40 patients with carotid artery atherosclerosis plaques were divided into 2 syndrome type groups, the plaques of phlegm and blood stasis type(n=20), blood stasis type(n=20) and a control group without atherosclerosis plaque(n=20).2,General condition of patients'record:age,sex,pul se pressure,heart rate,CTM symdrone type.3,ApplyⅤⅥtenhnoloy to measure the plaque:short axi s,thickest,single larger dissection,shoulder point, top point and the corresponding intimate pesk strain.4,To observe the carotid artery plaque VVI picture, to observe blood vessel inner membran movement vector value and change of direction. To analyze the different types of plaque shoudler,top point and the connesponding inner membran points against the no plaque reference response peak values.Results:1,Comparing with the no plaque carotid artery inner membran, the two dimension vector diagram of carotid artery ahterosclerosic plaque inner membran movement was chaos and asynchronous. The color anatomic M-mode and the reconstructed three-dimessional M-mode strain images also showed this obvious asynchronized strain distribution at the site of the carotid plaques.2,Short axis thickest single larger dissection:on plaque menbran 1-6 o'colck peak values has no statistical meaning(P>.05). Plaque membrance peak values has statistical meaing. That is, the plaque shoulder,top point peak values are singificantly higher than the reference counterparts. (P<0.05).3,The plaque shoulder,top point peak values of phlegm and blood stasis type are significantly higher.. (P<0.05). Conclusion:1,Both types of carotid artery atherosclerosis plaque shown 2 dimension vector value, direction and chaos. Phlegm and blood stasis type are more significant than the blood stasis type.2,During the heartbeat cycle, the plaque dissection response peak values shown multiple variations, different location shown different peak reaching time, reverse direction development was observed. The color anatomic M-mode shown confused colors, mixed red and blue color with no regular change pattern. Reconstructed three-dimessional M-mode strain shown plaque location inner membran responsive peak varied. In some location, movement lower or reverse development. The phlegm and blood stasis type is more significant.Quantitate Analysis:1,Both types of plaque short axis thickest single larger dissection, shoulder,top part inner membran location had increased responsive peak values, suggested the plaque vessel inner membran had higher stress and lower stability.2,The phlegm and blood stasis type responsive peak values were significantly higher than the blood stasis type., thus, the plaque stability is closely related with the TCM syndrome types.Research results suggest that:the two types of plaqued vessel inner membran's mechanical movements are not synchronised, their responsive peak values are different.This may recome the initial checking and quantifying indication, and provide imaging evidence to better TCM diagnosis.
