Objective1. To compare the 11C-MET PET/CT imaging with the clinical imaging methods, and to clarify the important diagnostic values of 11C-MET PET/CT in gliomas.2. To approach the expression of integrin betal in gliomas, and the possibilities of pathological diagnosis, disease monitoring and as a biomarkers of gliomas.3. By comparing the functional imaging with the tumor biological data, to clarify the correlation of 11C-MET PET/CT imaging and integrin beta 1' expression in gliomas'invasion, and then to reveal the biological behaviour of gliomas.Methods According to our study standard, a total of 31 gliomas patients were collected in Department of Neurosurgery, Tumor hospital of Yunnan Province, for the test group. and 5 The healthy check-up as a control. All the cases undergo the 11C-MET PET/CT and MRI, pre-and post-operation and radiotherapy, and then the results comparing and statistical analysis; Immunohistochemical staining also was employed post-operation, testing the expression levels of integrinβ1 in gliomas tumorous tissues, and sum the expression rule, statistical analysis in the end.Results1. In control group, the 11C-MET PET/CT imaging in brain without the limitations of radioiodine area, excepting the pituitary(figure 3). In test group (31 gliomas cases), Pre-operation,23 cases MRI positive, for 74.19% and 30 cases 11C-MET PET/CT positive, for 96.8%(P< 0.05). These data showed that 11 C-MET PET/CT was superior to MRI on pre-operative diagnosis. Post-opertation,3 cases'MRI imaging found anomalous signals,17 cases' 11C-MET PET/CT imaging found high uptake on the edge of tumor cavity(P<0.05), which considered the postoperative residual tumor. The results above indicated that 11C-MET PET/CT imaging is superior to MRI in postoperative residual tumor diagnosis. Among 20 gliomas patients with radiotherapy, MRI imaging detected 3 cases recurrent, and 17 cases indeterminated; PET/CT detected 9 cases recurrent,11 cases present radioactive necrotic lesions. The pathological diagnosis of 9 recurrent patients was gliomas after surgical resection, so the PET/CT was true positive (P<0.05). The results show that 11C-MET PET/CT is obviously superior to MRI in detecting radiation necrosis and recurrence of gliomas.2.10 normal brain tissues were collected from patients who underwent surgical decompression surgery in brain injury, in which the integrin beta 1 was negative by using Immunohistochemical staining; while in 31 gliomas tissues, the expression of integrin betal was:9 cases(-),9 cases(+),8 cases(++) and 5 cases (+++); All of which, the Integrin betal was positive in 6 low pathological level (40%) and 16 high pathological level(100%) gliomas. in low pathological level glioma positive 6 patients (40.0%), high level glioma 100.0% 16 cases (positive). So as the brain gliomas pathological level increased, the positive rate of integrin betal positive elevated.3. Detecting the different expression of integrin beta 1 between two groups, according to the pre-/post-operative tumor infiltration, distant metastasis or not by PET/CT imaging. In tumor infiltration cases, the positive expression rate of Integrin beta 1 is 94.1%(16/17), negative expression rate is 5.9%(1/17); In non-tumor infiltration cases, the positive expression rate of Integrin beta 1 is 42.9%(6/14), negative expression rate is 57.1%(8/14). By statistical analysis, integrin beta 1 and 11C-MET PET/CT are related in tumor invasion (r=0.542).Conclusion1. 11C-MET PET/CT imaging not only be able to accurately display the exact boundary, but also the proliferation state of tumors, provides comprehensive information for gliomas diagnosis, and helps to identify the recurrence and necrosis of glioma after radiotherapy.2. Integrin beta 1 were no expression in normal brain tissues, and expression in glioma cells, and it's level increased following the pathological level increased. Its expression mainly present in around the tumor infiltration area, which will be used to pathologic diagnosis, disease monitoring and as a biomarkers.3. Tumors, detecting by 11C-MET PET/CT, which has tumor weeks infiltration, distant metastasis and other biological behaviours, the SUV and T/N value were increased, the integrin beta 1 expression also increased. 11C-MET PET/CT imaging and the expression of Integrin betal are related in tumor invasion; Integrin betal has a correlation with glioma proliferation power and malignant phenotype of tumor.
