Objective:To comparatively analyze the CT characteristics and Pathological types of lung tumor with cysts in 25 patients and to improve the understanding of such kind of lesions and discuss the pathological basis of the formation of cystsMethods:From January 2008 to April 2011,25 cases of lung cancer with cystic lesions underwent multislice spiral CT in PLA General Hospital were selected. The clinical data-including age, sex, clinical symptoms, disease location, CT performance, clinical stage, pathological types were recorded. Correlation between the pathological section and CT images were performed in 12 cases.Results:25 patients including 14 males cases and 11 females cases.32-75years old, with mean age of 54±14.5 years old, Lesions were 17 cases were found in health examination,6 cases have cough, sputum or blood stained sputum, the other 2 cases have the malignant tumor on the other sites without obvious clinical symptoms of the respiratory system. In 25 cases,5 cases occurred in the left upper lobe,4 cases in the left lower lobe,7 cases in the right upper lobe,1 case in the right middle lobe,8 cases in the right lung lower lobe. All lesions in this group of are solitary, the size of lesion ranged from 1.0cm-7.3cm, lobulation could be seen in 22 cases (88.0%), spiculation in 17 cases (68.0%), pleural indentation in 19 cases (76.0%), vascular convergence in 19 patients (76.0%), cavitation or air bronchogram sign in 17 cases (68.0%).The lesion was predominant cysts in 6 cases, cysts and solid mixed lesions in 19 cases.There were 12 cases (48%)with stage I including 7 cases with stage la and 5 cases with stage Ib; 9 cases (36.0%)with stageⅡand all wereⅡa;3 cases (12.0%)with stageⅣ; precancerous lesions in 1 case (4.0%). Adenocarcinoma was found in 21 cases (84.0%), including_moderately-poorly differentiated adenocarcinoma in 11 cases, true bronchioloalveolar carcinoma in 1 case, adenocarcinoma mixed with bronchioloalveolar carcinoma in 9 cases; large cell carcinoma in 1 case (4.0%); metastatic tumor in 2 cases (8.0%), breast cancer and parotid adenoid cystic carcinoma; atypical adenomatous hyperplasia (AAH) in lcase (4.0%). Pathologically the cysts were formed due to emphysematous change with the alveolar wall were destroyed,and tumor necroses 2 nodules with cysts were followed 6 months,cyst disappeared.Conclusion:The most common type of lung cancer with cyst is adenocarcinoma, pathological basis of the fonnation of cysts mostly is emphysematous change due to the alveolar wall destruction. They have the typical signs of peripheral lung cancer in CT images,lung tumor with cysts,especially peripheral lung cancer.usully represent the early stages of tumor growth..
