Objective:To analyze the clinical features and therapy of patients with various degree of Bullous Pemphigiod, the objective of the thesis is to improve the diagnosis and therapy of BP. Methods:Clinical data of 60 cases of BP from January 2002 to December 2010 in the Department of Dermatology, Xinjiang Uinversity were collected. A retrospective study was made to those patients who divided into groups mild, moderate and severe according to the extent of skin lesions. Used statistical software package of SPSS 17.0 to analysis related data with various degree of BP. Results:there are 18 mild patients,17 moderate patients and 25 severe patients.60 patients,36 males and 24 females were enrolled. The male to female ratio of this group patients is 1.5:1,onset age from36 to 89 years old, with an average age was 67.93±11.63 years old. The gender and age is basically the same for different groups. The eruption of BP has pleomorphism. Majority patients appear tense blisters arising on apparently normal skin or on an erythematous plaque. Nikolsky 16.67% patients were positive. Mucosal lesions occurred in 31.67% of Patients. The more patient's condition was severe, the higher ratio of mucosae involved was. All Patients were examined by histopathology. Adopt direct immune fluorescence pathologic examination for 52 patients lesions,45 patients positive patients basement membrane IgG 67.31%, C3 76.92% sedimentary rate.46 routine IIF check, basement membrane linear fluorescent light 57.40%, rate of positive of heavy three groups was not statistically significant.The patients were mainly treated with glucocortieoid or the combination of glucocorticoid with immunosuppressive agents. The initial doses and maximum glucocorticoid doses needed to controllesions were different notieeably for different degree lesions. Conclusion:1. BP primarily affects old people, the gender and age difference is no statistical for different groups.2. BP complications of cardiovascular disease highest rate, but with illness aggravating merger, diseases of the nervous system ratio increases.3. The more patient's condition was severe, the higher ratio of mucosae involved was, and treatment needs to be large amount of glucocorticoid.4. The diagnosis of BP is based on histopathology and immunofluorescence. DIF mainly appear for the basement membrane with IgG, C3 linear fluorescence. rate of positive of antibody of lesions IIF detection with severity is not in parallel.5. System used glucocorticoid is still present treatment BP preferred solution. Disease severity is an important factor in influencing initial steroid dose, disease-controlled maximal dose, duration of steroid reduction during hospital izati on. In critically ill patients can be coupled immunosuppressive control BP illness reachs reduced glucocorticoid quantity, immunosuppressant choice also should emphasize individualized.
