Objective The study was performed to evaluate the curative effects, complications, recurrence of surgery or gamma knife for trigeminal neuralgia. Investigate the treatment and prognosis after surgery or gamma knife for trigeminal neuralgia. Methods Sixty-one surgery and eighty-six gamma knife patients of trigeminal neuralgia. were enrolled by means of telephone, letter and out-patient recheck who presented from December 1996 to June 2010. Application of chi-square test for all patients after 1 month,6 months and 12 months and follow-up at end times analysis of the situation of pain relief. Kaplan-Meier method was used to calculate cumulative pain relief curve. Results One hundred and two patients were followed for 3-151 months. Cumulative pain relief rates of 1,6,12month and follow-up at end times were 90.48%,95.24%,92.86% and 95.24% for forty-two surgery patients and 23.33%,83.33%,86.67% and 90% for sixty gamma knife patients respectively. The effect of two methods for treatment of trigeminal neuralgia difference was not statistically significant (χ2=2.053, p=0.152), pain relief rates in the short-term (in first month) demonstrated statistically significant differences(p<0.01), but pain relief rate in long-term demonstrated no significant differences(p>0.05).Conclusion For general situation is good, younger trigeminal neuralgia patients preferred MVD, and elderly cannot tolerate surgery can choose GK. MVD or GK treatment failure or relapses can be used again using MVD or GK. Pain relief rate of MVD in the short-term is higher than GK, both no obvious differences in long-term pain relief rate.
