The Clinical Analysis Of Autoimmune Liver Disease 218 Cases

Objective:To analyze and compare the three types of autoimmune liver disease about the clinical,biochemical,autoantibodies,therapies and curative effect, hope may give some helps to the clinical diagnoses and therapies.Methods:Clinical,laboratory and therapy data of 218 patients with autoimmne liver diseases (164 cases with autoimmune hepatitis,AIH;31cases with primary biliary cirrhosis,PBC ;23 cases with autoimmune hepatitis-primary biliary cirrhosis overlap syndrome AIH/PBC )were retrospectively analyzed,and compared with each other.Results:1)19 males and 199 females, ratio of gender is 1 to 10.51, 2)from 19 to 84, with a age of (51.67±12.78). 3) AIH in ALT increased significantly different with PBC and AIH/PBC,andγ-Glb different with PBC,PBC in GGT,ALP increased significantly different from AIH,and GGT from AIH/PBC. 4)Serum autoantibodies:(1)AIH patients ANA positive rate of 87.80 percent,SMA 7.93%,(2)PBC mainly positive in AMA 90.32%,AMA-M2 80.65%,(3)AIH/PBC overlap syndrome patients with ANA 95.65%, AMA 82.61%, AMA-M2 65.22%. There are litter parts of anti-SSA antibody positive, we can see the ALD is sensitively to have other autoimmune diseases. 5) The pathology of live accords with the principles, AIH tells the interface hepatitis, cholestatic hepatitis changing can be founded in the PBC cases,interface hepatitis with or not with cholestatic changes in AIH/PBC patients.6) therapies and curative effect:(1)The therapies of AIH are divided into two groups(with or without corticoid),only the countercheck of AST when patients discharged was significantly different between them. UDCA is the main therapy to PBC ,you can give the AIH/PBC patients with UDCA or add the corticoid.Conclusion: 1) ALD clinical performance of complex and varied,most of the patients with ALD are climacteric females.Due to the various clinical performances,the live biopsy examination,with the assistance of biochemical,autoantibodies are comparatively helpful to the accurate diagnosis. 2) Immunosuppressant if no contraindications of AIH,but we can see the daily medicines for hepatoprotecting and removing jaundice are also useful , UDCA mainly for PBC and AIH/PBC overlap syndrome. 3) We should work hard so we can take the diagnosis for easy,cut down the pay,effective treatment plan and fewest side effects .
