| The definition of ectopic pregnancy(EP)is zygote implanted out of the uterine cavity,so cervix pregnancy is a kind of ectopic pregnancy.Also it include tubal pregnancy,ovary pregnancy,abdominal pregnancy and broad ligament pregnancy.Although the mortality caused by EP decreased,it become a hot research pot for its incidence increased in our country and threatened the reproductive health.So many literature reported by foreign researchers are as below:The Americans reported that the incidence is still increasing in 2000. Norwegian find it increased before 1993. Swedish reported that the num.of EP/1000 pregnancy num. increased before 1990,but then to 1997,this rate decreased nearly 30%. Australia researchers finded that num.of EP/every 1000 birth num.decreased after 1993.In France,the total incidence of EP decreased from 1992 to 2002,but the rate of 'reproductive failure' of EP increased, while the 'contraceptive failure' of EP decreased.In our country,the incidence in different area includeing Zhejiang province,Sichuan province,Guangdong province and Yinchuan increased and also the age of the patient was on a trend of younger,especially primi pregnancy patient increased obviously.These fact threatened the female reproductive health and ring the alarm bell to our doctors that we should explore a more safety therapeutic method and strengthen to prevent EP happening.Based on the back ground above,we collected 948 ectopic pregnancy cases who were admitted to our department from Jan. 2007 to Dec.2009 and summarized the feature of incidence,age, gravidity and parity,the trend of therapeutic method.Then to give some suggestion for clinical practice.This study include two parts:①In terms of morbidity,we divided all the cases into 3 groups according to the year and compared the total number of cases and EP num./ total num. In terms of age,all the cases were divided into <20, 20～25, 26～30, 31～35, 36～40, >40,to compare the average age and the proportion. In terms of gravidity and parity,we have 3 groups:primi pregnancy,patient had not delivered,patient had delivered.②We divided all the cases into groups and then analysed by kinds of graph. Make a comparison about surgical therapy vs.medical therapy and laparotomy vs. Laparoscopy.The main results of this study are as below:1.Feature of the patient:①The number and proportion were 295(16.6%),330(16.0%),323(15.3%) respectively.It had no statistically significant(χ2=1.354 , P=0.508).②The average age was 30.31 years old,minimum was 16 years old,maximum was 50 years old.The peak age in 2007 and 2008 was 31～35 years old,disparate in 2009 was 26～30 years old.The average age of the patient below 20 years old was different between 2008 to 2009(p=0.028).The proportion of the EP patient below 30 years old per total EP number was different yearly(p=0.024).③Mean pregnancy times is 2.97, minimum was 1 times,maximum was 14 times.mean abortion times is 2.2,REP patient number is 95(10.02%).2.Therapeutic condition:①Pharmacotherapy cases were more than surgical cases and increased year by year.②laparo salpingectomy decreased gradually, Salpingostomy by laparoscope increased gradually.③The main Pharmacotherapy regime were single(47.7%) and multiple(26.0%) dose systemic methotrexate.④The effective rate of medical therapy and surgical therapy is different(100% vs. 93.42%,p=0.00).The time in hospital is more longer for medical therapy than surgical therapy,but the expenses is less. The time in hospital of laparoscope is shorter than laparotomy,the expenses and effective rate has no statistically significant between this two therapy.Conclusion: The proportion of ectopic pregnancy in Gynaecology was on a downtrend,but the number is increaseing yet.The age of the patient was on a trend of younger,especially in the patient below 30 years old.Abortion,REP,parity maybe are the risk factor for EP. Salpingostomy by laparoscope become a main surgical treatment. Pharmacotherapy regime increased year by year for its cost-effective characteristics.
