Clinical Analysis Between Completely Video-assisted Thoracoscopic Lobectomy And Conventional Thoracotomy

Abstract/Summary:
Objective : To evaluate the clinical value of completely thoracoscopic lobectomy for pulmonary diseases.Method : From July 2010 to March 2011, 47 cases which underwent completely thoracoscopic lobectomy (the Observed Group) and 47 cases which underwent conventional thoracotomy ( the Control Group) in the same time were compared in operation time, the number of lymph node, intraoperative blood loss, the chest tube conduction, postoperative chest tube time, postoperative hospitalization time, incision pain, complications, etc.Results : No operative morality occurred in either group. There were statistical significances between the Observed Group and the Control Group in operation time(180.1±40.3min vs 212.3±38.5min,p<0.05), intraoperative blood loss(135.1±35.8ml vs 170.2±28.1ml,p<0.05), the chest tube conduction(1077.4±265.1ml vs 1527.6±347.4ml,p<0.05), postoperative chest tube time(5.3±1.9d vs 6.9±1.7d,p<0.05), incision pain(5.6±1.7 vs 7.3±1.5,p<0.05),postoperative hospitalization time(6.2±1.8d vs 7.7±1.5d,p<0.05). There were no statistical significance between two groups in the number of lymph node(13.5±3.1vs 12.8±2.9,p=0.261). The total complications in the Observed Group were lower than the Control Group, but there was no statistical significance(10.6% vs 19.1%,p<0.05).Conclusion:Lobectomy for pulmonary disease by completely thoracoscopic lobectomy can reduce truma, accelerate rehabilitation, lighten pain, shorten the hospitalization time. Completely thoracoscopic lobectomy could accomplish the same efficacy in lymph node dissection for lung cancer cases. In a word, it is a safe and effective surgical procedure with minimal invasiveness, so it is worth spreading.
