Objective 1. to provide a experimental accordence to ensure the safeness of cavitas pelvis organs after selective arterial embolization in the bilateral internal iliac artery and the median sacral artery. 2. we propose to use gelfoam powder to embolize bilateral internal iliac and median sacral artery and then imitate the operation of sacral tumor measuring the change of intraoperative hemorrhage so that provide a experimental accordance for the determination of the best operation occation and the selection of gelfoam particle.Methods In the group A,B,C,D,E, there are sixteen common adult healthy dogs (weighing 10-13kg ,including fourteen male and two female ).Under the monitoring of DSA, in group A(3), embolize to the trunk of bilateral internal iliac artery and the median sacral artery;in group B(3), embolize to the trunk of bilateral internal iliac artery;in group C(3), embolize to the first branch of bilateral internal iliac artery and the median sacral artery;in group D(4), embolize to the trunk of unilateral internal iliac artery and the median sacral artery; in group E(3) embolize to the trunk of unilateral internal iliac artery with gelfoam power whose diameter between 50 and 150μm mixed with contrast media,and then observe the survival status and general,pathology status of bladder, rectum, sciatic nerve and sciatic muscles at the 3th day after embolization . In the group F,G,H, there are fifteen common adult healthy dogs (weighing 10-13kg,male ) which are divided into three groups ,The bilateral internal iliac and median sacral artery of twelve dogs are embolized using gelfoam powder as the embolized dogs in the group B, then we imitate the operation of sacral tumor to measuring intraoperative hemorrhage ,observing pathological alteration of pelvic organ at different times–one day(groupF,5) ,two days(groupG,5) ,three days(groupH,5 ) , after embolization.Results In the 7 groups, seven dogs ( three dogs in group A,three dogs in group C and one dog in group D )die in the 48hours after embolization ,and the others ( three dogs in group B;three dogs in group D; three dog in group E;fifteen dogs in the group F,G,H) satisfactorily survive until conclusion of this experiment ,about half of that recovery normal defecation. The rectum nigrities, necrosis; the bladder present lamellar obfuscation and lamellar hemorrhage edema;there is much urinary volume in the bladder in the dead dogs( in the group A,group C,group D).The secrum of living dogs(in the group B,D,E,F,G,H) have no obvious alteration;the bladder only manifest light edema, and the urinary volume is littler in many dogs. The rectum, bladder of dead dogs (in the group A,group C,group D) manifest cell disruption, generous inflammatory cell infiltrate and epithelial cells ablating; the rectum, bladder of the living dogs( in the group B,D,E,F,G,H) manifest light inflammatory cell infiltrate and edema at the 3th day after embolization.In the group F,G,H, the mean intraoperative hemorrhage at the first,second ,third day respectively is 225.00±47.21g, 253.20±55.66g,346.80±67.76g,to compare the intraoperative hemorrhage between the first day and the second day,P>0.05, to compare the intraoperative hemorrhage between the first two days and the third day,P<0.05.When we use gelfoam power whoes diameter is between 50 and 150μm to embolize bilateral internal iliac and median sacral artery ,The smallest mebolized artery are arteriole whoes diameter are about 50μm ,but mebolized artery mainly concentrate on arterioles whoes diameter are between 100 and 200μm .The sciatic nerve and sciatic muscles of every group have no obvious alteration except for light edema. Conclusion 1. When embolize the internal iliac artery and median sacral artery with gelfoam power whoes diameter is between 50 and 150μm, to ensure the safeness of pelvis organs, the embolized artery cannot exceed the first branch when embolized the three arteries at the same time, or reserve at least unilateral internal iliac artery when embolized to the trunk. 2.The optimal period for the operation of imitation sacral tumor is 48 hours after embolism with this gelfoam power,diminute the diameter of gelfoam particles can increase embolism effectiveness and implications.
