BACKGROUND:Lung cancer is one of the most common malignant tumors in the world today,the mortality rate of lung cancer has been the top amongst all kinds of malignant tumor. Non-small-cell lung cancer (NSCLC) accounts for about 80%-85% of all lung cancers diagnosis, and the majority of people diagnosed with NSCLC have advanced disease, of which about 25%-30% is locally advanced, about 40%-50% have distant metastasis, thus lost the opportunities for surgical resection, palliative chemotherapy is the main treatment for them. At present, platinum-based chemotherapy which combined the third-generation drugs (gemcitabine, vinorelbine, taxanes, etc.) is the standard treatment for advanced NSCLC. However,the response rate of chemotherapy was only 30%-40%. Although some patients who with the same histology type,the same clinical stage and treated with the same therapy or chemotherapy plan, the outcomes turned out to be different, tumor drug resistance, relapse and metastasis is the main reason for treatment failure. It is well known that drug resistance,recurrence and metastasis of tumor is a complex process involving multiple factors. The theme of ASCO in 2009 is "personalizing cancer care". So, according to personality characteristics of tumor cells to select individualized treatment methods, can improve efficacy, reduce side effects. The examination of some molecular biomarkers may instruct the treatment and benefit the patients largely. K-ras,β-tubulin-Ⅲ,bFGF was closely related to the occurrence,development, diagnosis and prognosis of NSCLC.K-ras is closely related to lung cancer, is considered one of the key genes of canceration , its high expression signifies that cancer cell has strong capacity of proliferation and invasive ,high risk of tumor recurrence and death, and is the performance of highly malignant cancer, its mechanism and the relationship with NSCLC has been widely accepted and recognized.Microtubules ,which is arrangement vertically by 13 fibrils and formed a columnar hollow structure,constitute the main constituted component of the cytoskeleton.Tubulin hasα,βtwo isoforms,each of the fibrils is tubulinαandβheterodimeric assembly into the polymer, with the function of maintenance the cell shape,exchange material, transfer information, participation mitotic. There are 6βtubulin isoforms in human cells, of which,β-tubulin-Ⅲis the most closely related to the sensitivity of chemotherapy drug acting on microtubules.Basic fibroblast growth factor plays an important role in the regulation of angiogenesis, it can stimulate division and proliferation of endothelial cell, with the function of promoting mitosis and inducting angiogenesis, and closely related to tumor differentiation, invasion, growth, metastasis and tumor angiogenesis, and play a role in many tumor angiogenesis.This study investigated the expression of K-ras,β-tubulin-Ⅲ, bFGF in peripheral blood,lymph nodes and tumor tissues in patients with non-small cell lung cancer, and the relationship between the expression and clinicopathological characteristics.And also analyzed the correlation among the expression of K-ras,β-tubulin-Ⅲ, bFGF in peripheral blood, lymph nodes,tumor tissue in NSCLC patients and their significances, and then,to guide the early diagnosis,individualized treatment and prognosis of NSCLC.METERALS AND METHODS:We chose 45 patients of NSCLC with complete clinical material in our hospital,4ml peripheral blood was taken from each patient in this groups before operation, the lymph nodes and tumor tissues of these patients were obtained during operation. We detect the expression of K-ras,β-tubulin-Ⅲ,bFGF mRNA in peripheral blood,lymph nodes and tumor tissues with real-time PCR. All tests were the two-sided. All statistical testing was conducted at the 0.05 level and SPSS software version 17.0 was used.RESULTS:1. The expressions of K-ras in peripheral blood,lymph nodes and tumor tissues in NSCLC patients were associated with smoking, pathological type,the conditions of lymphatic metastasis(P<0.05), were not associated with age, sex, tumor size, clinical stage and differentiation degree(P>0.05).2. The expressions ofβ-tubulin-Ⅲin peripheral blood,lymph nodes and tumor tissues in NSCLC patients were associated with pathological type,the conditions of lymphatic metastasis and clinical stage(P<0.05), were not associated with smoking, age, sex, tumor size, differentiation degree(P>0.05).3. The expressions of bFGF in peripheral blood,lymph nodes and tumor tissues in NSCLC patients were associated with smoking, the conditions of lymphatic metastasi and clinical stage s(P<0.05), were not associated with age, sex, pathological type, tumor size, differentiation degree(P>0.05).4.Linear correlation analysis revealed that the expressions of K-ras,β-tubulin-Ⅲ, bFGF in peripheral blood was positively correlated with their expression in lymph nodes and tumor tissues in NSCLC patientsCONCLUSION:The expressions of K-ras,β-tubulin-Ⅲ,bFGF relate to lymph node metastasis in patients with Non-small cell lung cancer,and K-ras, bFGF expressions are smoking-related, K-ras,β-tubulin-Ⅲexpressions are pathological-related,β-tubulin-Ⅲ, bFGF expressions are clinical stage-related.Therefore, the detection of K-ras,β-tubulin-Ⅲ, bFGF expressions are propitious to guide diagnosis and determine whether there is lymph node metastasis,guide clinical staging and treatment.In addition, the expressions of K-ras,β-tubulin-Ⅲ, bFGF in peripheral blood was positively correlated with their expression in lymph nodes and tumor tissues in patients with non-small cell lung cancer. Through the analysis, we draw the conclusion: the detection of K-ras,β-tubulin-Ⅲ, bFGF in peripheral blood may take the place of the detection in lymph nodes and tumor tissues, so we can provide a quicker and easier examination method for the patients in need.
