ARSTRACT Objective To study the advantages of ultrasonically activatedscalpel(UAS)in Axillary lymph node dissection compared with electrosurgery.Methods Modified radieal mastectomies were performed in 58 cases with breast cancer of breast professional group in our department between Jan 2009 and Dec 2009.Patients were divided into two groups:in UAS group(n=29),axillary lymph node dissection was performed by using the Ultracision Harmonic Scalpel,in electrosurgery group(ELC,n=29)the operations were performed by monopolar electric scalpe1.Age.sex and condition of the two groups were similar.Major indexes to observe and compare include:the daily draining volume postopratively from axillary drainage tube.draining time and postoperative complications such as subcutaneous hematocele.Results Daily drain volume postopratively of the UAS group were more than the electrosurgery group,and their differences are not significant.The numbers of days with the axillary tube of the UAS group were slightly more than the electrosurgery group,and their differences are not significant.We further grouped the patients according to their body mass index(BMI), BMI greater than or equal to 25 of the patients were overweight, and BMI less than 25 were non-overweight. In overweight patients, the daily draining volume postopratively from axillary drainage tube and draining time of UAS group were more than ELC group, and their differences are not significant.In non-overweight patients, the daily draining volume postopratively from axillary drainage tube of UAS group were more than ELC group, and draining time of UAS group were less than ELC group.And their differences are not significant.There were 3 cases with subcutaneous hematocele both of the UAS group and the electrosurgery group,there was no differences.The overall operation process of clearing of axilla could performed wih UAS.Injuries of axillary vesse1,subscapular vessel,long thoracic nerve and thoracodorsal nerve did not happen.Conclusion Using UAS in modified radical mastectomies, there is clear vision intraoperative, and it's easy to play.It's used in clearing of axilla without injury axillary vessels and nerves. However, in reducing postoperative drainage and and the draining time has yet see the advantages.The Ultrasound—propelled incision and hemostasis were satisfactory, and can be used to clear axilla.It seems to a new,excellent operation energy with more superiorities than ELC.It can be applied widely in radical masteetntnies.
