Objective To study the therapeutic effect of the atrial fibrillation suppres sion pacing (AFSP) in the patients with bradycardia-tachycardia syndrome by following-up study for a moderate to long term. Methods Twenty one patien ts with bradycardia-tachycardia syndrome (male=7, female=14, age=69.71±8.4 92y) had been followed up since the AFSP pacemaker were implanted. The fol lowing up was done by clinical observation,12-lead HOLTER (12-HOLTER) monitoring and pacemaker detecting. The following up data for a period of 5.4 8±2.52y were analyzed to evaluate the therapeutic effect of AFSP to atrial fibr illation. Results (1) The medicine of (3-blocker was taken by all of the 21 pa tients and the symptom was improved (the symptom disappeared for 8,38.1% and being improved for 13,61.9%) since AFSP pacemaker were implanted. Per sistent atrial fibrillation was observed in 5 patients (23.81%) and 1 died in the following up. (2) It was found in the 12-HOLTER following up that the atria 1 premature was decreased significantly (2,235.19±957.51beats/24h,1,086.33±579. 4beats/24h,P<0.05). The atrial loads such as the beats of paroxysmal atrial fibri llation (PAFB), the persistent time of PAF (PAFT) and the percentage of PAFT (PAFT%)were decreased significantly (PAFB:26,463.81±37,742.68beats/24h,11, 652.19±23,700.75beats/24h;PAFT:6.96±9.85h,3.23±6.81h; PAFT%:30.43±9.40%,1 1.43±4.71%; P<0.05) since the pacemakers were implanted. (3) The following up on echocardiogram showed that the left atrium and left ventricle were not e nlarged (39.57±4.26 mm,39.29±0.70 mm; 47.24±4.76mm,48.10±1.14mm; P>0. 05); and the LVEF was significantly increased (63.85±2.55%,65.10±2.48%, P< 0.05) since the AFSP pacemakers were implanted. (4) The following up by the pacemaker detecting showed that the percentage of artral pacing was increased (56.28±7.12%,73.00±7.03%,P<0.01) and the events of Automatic Mode Switch( AMS) were decreased significantly (3,413.52±1,802.06,2,277.10±1,441.27,P<0.05) since the AFSP pacemakers implantation. Conclusions (1) The AFSP combine d withβ-blocker could be used effectively to treat bradycardia-tachycardia; (2) The AFSP combined withβ-blocker could be used effectively to control atrial fibrillation and decrease the atrial fibrillation load; (3) The following up by 12-HOLTER monitoring and AFSP pacemaker detecting could be used to evaluate the therapeutic effect of the AFSP combined with P-blocker...
