Using the diagnostic criteria of TCM syndromes included in the diagnosis and treatment programs for lung cancer written by specialists in the department of health policy in china, the TCM syndromes were judged based on their tongues, pulses and symptoms, and the relationship between TCM syndromes and first symptoms in untreated patients with adenocarcinoma of lung and stage IV was also studied. In addition, the clinical features including metastasis sites,levels of tumor markers in blood were observed in this study.[Results]The main TCM syndromes of this group of patients includes Qi stagnation and blood stasis (29 patients), deficiency of Qi and Yin (22 patients) and phlegm-heat obstructing lung (21 patients).The relationship between TCM syndromes and first symptoms in untreated patients with adenocarcinoma of lung and stage IV as following:The main TCM syndrome is phlegm-heat obstructing lung in pat ients whose first symptoms are presented with non-infection related lung symptoms such as cough, chest distress and shortness of breath;For patients whose first symptoms are presented with hydrothorax related cough, shortness of breath and asthma, the main TCM syndromes are Qi stagnation and blood stasis and deficiency of Qi and Yin, next is the syndrome of phlegm-heat obstructing lung;Qi stagnation and blood stasis is the main TCM syndrome in patients whose first symptoms are caused by bone metastasis.The pleura metastasis are most commonly presented in untreated patients with adenocarcinoma of lung and stageâ…£(29/48,60.42%), other metastasis sites are bones, lymph nodes, lungs, brain and liver metastases in this group.The CEA positive rate is 72.92%, and CA125 positive rate is 75% in this group of patients. There are 29 patients (60.42%) in which both CEA and CA125 are elevated at the same time.[Conclusions]The main TCM syndromes in untreated patients with adenocarcinoma of lung and stage IV are Qi stagnation and blood stasis, deficiency of Qi and Yin, and phlegm-heat obstructing lung.
