Autologous Chondrocyte Implantation And Microfracture In Cartilage Repair Of The Knee: A Systematic Review

BACKGROUND: Cartilage defects of the knee is a common disease in the clinical medicine and it is often found in teenagers who exercise a lot. Nowadays the common and traditional way to treat it is microfracture(MF). Since the G?teborg university in Sweden finished the first autologous chondrocyte implantation (ACI) in 1987, around the world people have finished ten thousands of the operation like this. And many facts proved that this method is worthy .The relative differences in effectiveness of ACI and MF are still unclear.OBJECTIVE: It is the objective of this study to systematically review the literature on autologous chondrocyte implantation compared to microfracture by clinical outcome and the quality of the repair tissue, including an assessment of the validity of these findings.METHODS: The online databases PubMed, EMBASE, Cochrane Controlled Trial Register, CBM,CNKI and VIP were searched. Controlled trials comparing autologous chondrocyte implantation with microfracture of cartilage repair. Data on clinical outcome and the quality of the repair tissue was abstracted in duplicate. Study validity was assessed by individual components.The date of each study was analyzed by RevMan5.0.RESULTS AND CONCLUSION: Five studies involving 436 patients were included Studies. Comparing autologous chondrocyte implantation with microfracture have a higher quality and show no differences in clinical outcomes, but suggest better results in tissue quality. Among the included studies there is much inconsistency in methodological quality and findings. Future studies will be needed to answer the question of benefits of autologous chondrocyte implantation compared to microfracture, and could profit from addressing and avoiding the problems seen in this group. Finally conclusions concerning long-term effects are still difficult.
