Objective With the purpose of offering proof that medical adhesive as mesh fixtion can be an optional method in clinical practise , we set this experiment to compare the impact on mesh shrinkage and assess validity of fixation , viscous force of fixation , and affection on mesh when using medical adhesive and sutures as mesh fixation .Methods Part one of the experiment : Two lesions of 1cm×1cm were dissected with a scalpel blade in the right and left abdominal rectus muscles respectively . And then a abdominal hernia modle was completed . With either medical adhesive or sutrues as mesh fixation , a mesh (size : 2cm×2cm) was implanted in each side . Rats were sacrificed in deep anaesthesia on the 60th day post operatively , and then calculate the shrinkage area of mesh and shrinkage rate . At the same time , the integration of mesh and abdominal tissue was observed .Part two of the experiment : A lesions of 1cm×1cm was dissected with a scalpel blade in the middle of abdominal rectus muscles . A mesh which size is 2cm×2cm was implanted with medical adhesive as mesh fixation . The rats were killed in deep anesthesia on the 3 minutes , on day 10 , on day 30 and on day 60 postoperatively . Then calculate the viscous force between the mesh and muscles .Result The result of part one : After two months , as the side in which meshs were fixed by medical adhesive , the mean area of mesh was 3.85±0.12(cm2)with a shrinkage rate of 3.75% . The other side in which meshs were fixed by sutures , the mean area of mesh was 3.43±0.33(cm2) with a shrinkage rate of 14.25% . The difference of date of the two group was significant . It was illustrated that the shrinkage area of the mesh fixed by medical adhesive was smaller than the one of the mesh fixed by sutures . Meanwhile , the pathological result of both side was similar with each other , and it meant that using medical adhesive as mesh fixtion did not hamper the integration of mesh and abdominal tissue .The result of part two : the viscous force that were calculated on 3 minutes , on day 10 , on day 30 and on day 60 were following as : 6.18±4.13(N),5.89±4.08(N),13.31±5.14(N),26.29±10.00(N)respectively . Compared to the max tension reference value ( 2 N/cm2 ) of abdominal wall ,the viscous force at each time point of surveying were significant . And it meant that medical adhesive could fix the mesh utility since the time 3 mins after it was used . Analysing the viscous force value at each time point with one-way analysis of variance (ANOVA) , there was a difference between each of them . So a boost LSD-t test was required . And then , the result was that : as one by one comparison , all group have a difference except for the comparison between the date on 3 min and the date on day 10 . The regression equation of time point and viscous force was y=-4.03+6.81x , the correlation coefficient is 0.747(P<0.0001). So it was demonstrated that there was a positive correlation between time and viscous force . And simply that is : longer time , stronger force .Conclusion : The shrinkage area of the mesh fixed by medical adhesive was smaller than the one of the mesh fixed by sutures . Meanwhile , medical adhesive does not hamper the integration of mesh and tissue ;Using medical adhesive as mesh fixation is a simple, effective and safe method . And as the time goes by , the viscous force is stronger than previously .
