Objective:To study the effects and indications of dynamic hip screw (DHS) and proximal femoral nail (PFN) as treatments for intertrochanteric femur fractures. Materials and Methods:Twenty nine cases of femoral intertrochanteric fractures in our hospital from 2006.6 to 2009.6 were divided into DHS group and PFN group according to different internal fixation. All of the cases in the two groups were combined without osteoporosis(Singh index overâ…¢).All patients were treated surgically, nineteen with DHS including Tronzo and Evens typel 9 cases, typell 6 cases, typelll 4 cases and ten with PFN that all were unstable intertrochanteric fractures. Result:All patients were followed up about six months to two years. The average fracture healing time was no obvious difference between DHS and PFN. There were obvious differences in diseased site and Harris Hip Score(HHS) between DHS used for stable intertrochanteric fractures and unstable intertrochanteric fractures. The effects of PFN used for unstable intertrochanteric fractures were satisfactory.Conclusion:1. Both DHS and PFN can be good treatments for the intertrochanteric fractures in the elderly, but the operation indication should be chosen strictly.2.There are better effects of DHS used for stable intertrochanteric fractures than unstable intertrochanteric fractures. PFN is more suitable than DHS for unstable intertrochanteric fractures.3. There should be more research on the operation time, blood transfusion during and after operation and so on. To chosen suitable materials was difficult for the cases of serious osteoporosis.
