Objective:Through the experimental studies and clinical observation to study the security of Avastin in the eye application.Methods:Injected different dosage of Avastin to rabitts vitreous chamber (1.25mg/0.05ml,2.5mg/0.1ml,5mg/0.2ml) and anterior chamber (1.25/0.05ml). Corneal endothelial microscopy,OCT and ERG examination were treated before and after 7 days injection. The eye-balls were taken and tissue structure of cornea and retina were observed under optical microscope after 7 days injection. Statistical Analysis the data of corneal endothelial cell density, area coefficient of variation,OCT and ERG b-wave amplitude. Selected 29 patients (30 eyes) with diabetic retinopathy given Avastin 1.25mg/0.05ml intravitreal injection. Before and after 3 days,1 week,1 month,3month injection, respectively examine there corneal endothelial to observe the selected area of corneal endothelial changes in the indicators.In order to observe the ERGb wave amplitude and peak time changes and retinal edema of the ministry of the yellow spot,ERG,OCT and fundus fluorescein angiography examination were taken before and after 1month injection.Results:Corneal endothelial cell significantly density and cell size coefficient of variation significantly increased in the group of injection 5 mg Avastin to vitreous cavity and group of injection Avastin to anterior chamber.1.25mg group and 2.5mg was no significant change. corneal endothelial cell density reduced and size coefficient of variation increased in the group of Anterior chamber injection as compared with the other groups the difference was statistically significant(p<0.05). After intravitreal injection,5mg group measured eye-OCT retinal thickness significantly thicker, the difference was statistically significant (p<0.05),the other groups no significant change, the difference was not statistically significant(p>0.05). ERG b-wave amplitude significantly decreased in all experimental groups after injection to intravitreal of rabbit eyes. ERG b-wave in 5mg group was significantly lower than the other three experimental groups and control group. Transmission electron microscopy showed that anterior chamber injection set of experimental corneal endothelial cells, sparse organelles in the cytoplasm, showing that short and expanded endoplasmic reticulμm, epithelial damage apparent connection. The other set of experiments eyes were better than the anterior chamber injection group.5mg set of experiments focus most of the optic nerve fibers of the cross section of vacuolization, mitochondrial swelling; a more normal synaptic connection structure very little, and some despite neurotransmitter vesicles, but the post-synaptic membrane HDF missing, the other group did not apparent anomaly Clinical observations show that all patients with corneal endothelial cells in the test showed no significant change, there was no statistically significant difference (p<0.05). ERGb waves and no significant change in peak time difference was not statistically significant (p<0.05). The yellow spot department was reduced, edema is statistical significance (p<0.05). new blood retina significantly to retreat visibly reduced and leakage.Conclusion:The intravitreal injection of Avastin below 5mg in the short term animal retinal structure and function of nerve conduction had no significant effect. But 5 mg group have appeared optic nerve fibrous cross section vacuole melt, mitochondria's swelling, the cavity fault no easy to inject only inferior dosages of Avastin pointing out the vitreous body is big, gap time had not exchanged shortly, to cut down or avoid the poisonous side effect that Avastin accμmulation produces to retina. Clinical intravitreal injection Avastin1.25mg is safty.
