Clinical Analysis Of 770 Cervical Diseases Patients With Conization

ObjectiveTo explore the indications , advantages and disadvantages of cervical conization.And to explore whether there are over-treatment in the veriaty of cervical disease.MethodsA retrospective analysis in 770 cases of patients with cervical conization of the Second Hospital of Jilin University from January 2009 to December 2010. A retrospective analysis was done before the examination, preoperative diagnosis and postoperative pathology results of the cases with cervical conization。What we have done is to summarize the number of diagnostic conization and therapeutic conization, to summarize the number of loop electrosurgical excisional procedure and cold knife conization so that we can analyze the reason of doing LEEP and CKC ,and evaluate whether the treatment consistent with the indications of cervical conization.According to the treatment guidelines of LEEP and CKC, Evaluate the the proportion of the cases of compliance and uncompiance of the therapeutic indications.And summarize the the cases of uncompiance of the therapeutic indications so that we can evaluate whether there are over-treatment in the veriaty of cervical disease and analyze the reasons of over-treatments.ResultsA total of 770 cases,there are 31 cases of diagnostic conizations and 752 cases of therapeutic conizations.And there are 700 cases of loop electrosurgical excisional procedures and 70 cases of cold knife conization.There are 408 cases which have cervical biopsy before cervical conization the pathology.Of which there are 33cases(8.1%) including 15 cases(5.5%) of benign cervical diseases and 18 cases of cervical intraepithelial neoplasia upgrade after cervical conization. There are 344 cases without cervical biopsy before cervical conization the pathology which only have TCT or colposcopy or Visual diagnosis.Of which there are 278 cases of Chronic cervicitis and 24 cases Cervical polyps and 7 cases of Nessler cervical cyst and 6 cases of CINⅠand 4 cases of CINⅠ-Ⅱand 2 cases of CINⅡand 9 cases of CINⅡ-Ⅲand 11 cases of CINⅢand 3 cases of Invasive cervical cancer.According to the indications of LEEP and CKC,in the 408 cases which have cervical biopsy before cervical conization there are 275 cases which does not meet the therapeutic conization indications,( 275/408,67.4%).and the rest 133 cases meet the therapeutic conization indications,( 134/408,32.6%).And there are 338 cases of LEEP, including 74(21.9%) cases which does meet the therapeutic conization indications.and there are 70 cases of LEEP, including 59(84.3%) cases which does meet the therapeutic conization indications.ConclusionThe role of Conization in diagnosis and treatment of cervical disease is very significant. there are strict indications in conization and it is not universal knife treatment of cervical disease.Over-treatments and side-effects exist in using conization.over-treatment can cause both Physical and psychological damages of patients.The causes of Over-treatments include both patients and doctors,and most of which are the wrong views of the cervical diseases especially the benign cervical diseases.We should improve the medical level and choose proper and the special treatment to each cercical disease which can benefit to the the physical and mental health.
