Epilepsy is excessive synchronous discharge of neurons caused by repeated recurrent seizures as features of chronic recurrent episodes of transient cerebral dysfunction syndrome, is a common disease of the nervous system. Drug refractory epilepsy in patients with epilepsy accounted for 20% to 40%, in which the mesial temporal lobe epilepsy common. Patients with longer duration, seizure frequency, medication poor, seriously affecting the quality of life, to the heavy burden on family and society. Major surgical theraphy methods for MTLE have craniotomy, stereotactic thermal coagulating amygdalotomy,stem cell transplantion and gamma knife surgery. Gamma Knife is an important means of treatment of epilepsy, how to precisely locate epileptogenic focus is the key to the success of gamma knife treatment of epilepsy. EEG stereotaxic system to find and locate, which includes a 128-lead EEG video synchronization monitoring system, the dipole analysis, EEG and MRI image fusion technology, can significantly improve the positioning of epileptogenic focus accuracy. Methods:from January 2004 to December 2007,54 patients with mesial temporal lobe epilepsy were treated at 363 Hospital Gamma Knife treatment centre.(32 cases of male patients,22 cases of female patients, aged 8 to 53 years, mean age 25 years old.) To evaluate the effect of treatment by A prospective research methods, the follow-up observation, and statistical analysis using statistical software to compare it with sex, age, onset age, treatment dose, dipole localization results, follow-up time and other related factors.Results:54 patients were included in this group of patients, The average peripheral dose of 12Gy (8-16Gy), followed up 1 to 6 years, an average of 4.2 years. treatment efficacy according to international common evaluation criteria: 21 cases of gradeâ… (38.9%),13 cases of gradeâ…¡(24.1%),9 cases of gradeâ…¢(16.7%),11 cases of gradeâ…£(20.3%); 43 cases of the treatment site is consistent with the EEG, Dipole localization results,21 cases of gradeâ… (48.8%),10 cases of gradeâ…¡(accounting for 23.3%),6 cases of grade (13.9%),6 cases of gradeâ…£(13.9%); The remaining 11 cases are inconsistent, No case of grade I,3 cases of grade 11(27.3%),3 cases of gradeâ…¢(27.3%),5 cases IV (45.4%); no deaths.Conclusion:The long-term follow-up results of EEG stereotaxic system guided gamma knife treated mesial temporal lobe epilepsy showed good seizure control rates and low complications. The results showed good therapeutic effect when EEG coincide with dipole localization results; Conversely, this procedure should not be proposed to patients in whom EEG results suggest that the epileptogenic zone is not restricted to mesial temporal structures and do not coincide with dipole localization results, the first choice treatment is temporal lobe surgery, intraoperative cortical electrode monitoring verification Epilepsy source area. Therefore, before Gamma Knife treatment must be strictly selected patients, there should be sufficient basis to determine targets, strictly control the dosage, the volume to increase efficacy and reduce complications.
