Objective To explore the value of CTPA in evaluation of the cardiovascular parameters ofacute pulmonary embolism before and after treatment.To purpose multi-angle, multi-dimensionalobservation of the degree thrombosis and the form of emboli with majorized thethree-dimensional reconstruction of post-processing technology.Method A total of 118 patients clinically suspected as PE were enrolled in this study byCTPA were studied.The patients were without PE(n=40);SPE group server PE(n=42);and NSPEgroup non-severe PE(n=36);The patients were treated by fibrinolysis and by heparin in NSPEgroup.CTPA were performed after the treatment for 14d.Based on the diagnosis ofCTPA,difference of cardiovascular parameters among 3 groups was analyzed;The cardiovascularparameters were observed before and after treatment. The total of 118 patients underwentThree-dimensional reconstruction, including: multiplanner reformation; maximum intensityprojection; volume rendering.Result CT obstruction index in patients with SPE group was higher than that of patients withNSPE group .The cardiovascular parameters before treatment included the maximum diameter ofleft and right ventricular short axis(cm),left and right ventricular maximum area(cm2),the rightand left ventricular wall thickness and interventricular septum thickness(cm),azygos vein andsuperior vena diameter(cm),main pulmonary artery,right and left pulmonary arterydiameter(cm),from left ventricular septal rate,inferior vena cava or azygos vein reflux rateshowed significant difference between 3 groups.The correlation between CT obstruction indexsand cardiovascular parameters was significant Spearman's rank correlation coeffient was higherbetween right pulmonary artery diameter and CT obstruction index.The cardiovascularparameters between patients with SPE group and NSPE group before and after treatment showedsignificant difference.Conclusion CTPA not only can diagnosis of pulmonary embolism also can evaluate the degreeof pulmonary arterial obstruction and the change of right heart function,and then reflect theprognosis and observe curative effect. Majorized three-dimensional reconstruction made a betterdisplay for the degree of pulmonary embolism and the embolic form, providing the basis forclinical treatment.
