Objective To investigate values of WB-DWI in differential diagnosis of Multiple osseous metastases and Multiple myeloma. Methods 57 cases with multiple bone tumors and 12 cases normal volunteers underwent WB-DWI. Furthermore, the signal characteristic and ADC values range of the thoracic vertebra, lumbar and ilium were analyzed and measured, respectively. In additional, the detection frequency for searching primary tumors in patients with multiple osseous metastases was calculated.①Multiple osseous metastases and Multiple myeloma demonstrate abnormal high signals in WB-DWI.②The ADC values of thoracic spines in volunteers% multipile bone metastases and multiple myeloma were (0.518±0.104)*10-3mm2/s, (1.388±0.324)* 10-3mm2/s, (0.835±0.087)* 10-3mm2/s. The ADC values of lumbar spines in volunteers, multiple bone metastases and multiple myeloma were (0.488±0.134)*10-3mm2/s, (1.317±0.215)*10-3mm2/s; (0.832±0.080)* 10-3mm2/s. The ADC values of ilium in volunteers, multiple bone metastases and multiple myeloma were (0.546±0.146),10-3mm2/s, (1.546±0.333)*10-3mm2/s, (0.836±0.095)* 10-3mm2/s.The ADC values of multiple myeloma and multiple osseous metastases were higher than that of normal bone.Meanwhile, The ADC values of multiple osseous metastases were higher than that of multiple myeloma..The difference was significant in ADC values between multiple myeloma and multiple osseous metastases (P<0.01). the differences of ADC values between thumbar spines, lumar spines and ilium of normal volunteers and multiple myeloma were statistically significant(P>0.05). the differences of ADC values between thumbar spines and lumar spines of multiple osseous metastases were statistically significant, but, the differences of ADC values between thumbar spines, lumar spines and ilium of multiple osseous metastases were significant(P<0.01).③The detection frequency for searching primary tumors in patients with multiple osseous metastases was 74.5%(35/47). diagnosed by quantitative analysis with ADC value. The WB-DWI can demonstrate primary tumors, which also can help to differentially diagnose multiple osseous metastases and multiple myeloma.
