| Objective:The authors compared the efficacy and tolerability of ziprasidone and olanzapine in the treatment of inpatients with first-episode schizophrenia.Method:In this 6-week, randomized, opened, prospective study, the ratio is 1:1,efficacy measures included Clinical Global Impression (CGI) and severity scale scores (SI) and Positive and Negative Syndrome Scale (PANSS). Safety and tolerability assessments included Extra pyramidal Side Scale (RESES), biochemical indexes, fasting glucose, Fasting-insulin, prolactin parameters, electrocardiography and monitoring of vital signs and body weight.Results:In this trial,63 patients were randomly assigned to receive ziprasidone (N=31) or olanzapine (N=32).The overall mean daily doses were 123.1±52.8 mg for ziprasidone and 17.5±7.07mg for olanzapine. Both antipsychotics were efficacious in reducing the scores of PANSS and CGI-S, The two treatment groups did not differ significantly in efficacy measures at endpoint. Both agents were well tolerated. Body weight, body mass index, low-density lipoprotein, total cholesterol, triglycerides, and high-density lipoprotein, fasting blood-glucose, Fasting Insulin significantly increased with olanzapine but not with ziprasidone. The level of RESES, prolactin and QTc gained significantly increasing in inpatients with ziprasidone not with olanzapine. All between groups comparisons of these variables were significant. No patient in either group exhibited serious adverse event, such as severe arrhythmia, hypohepatia, kidney failure, granulocytopenia.Conclusions:During 6 weeks'treatment, ziprasidone and olanzapine demonstrated comparable antipsychotic efficacy. In conventional dosages, the results showed that two groups have better safety and tolerability. Differences favoring ziprasidone were observed in metabolic parameters such as body weight, blood lipid and blood glucose, EPS, QTc and the level of prolactin in favor of olanzapine.
