| Objective:The main objective of this study is to explore the effect of An-Chang-Yu-Yang Decoction in curing the recrudesced or persisted type ulcerative colitisin in active state through clinical and experimental observation, elucidate the possible mechanism of this decoction on Ulcerative Colitis, so as to provide theoretical basis for the treatment of Ulcerative Colitis.Methods:sixty patients with the recrudesced or persisted type ulcerative colitis in active state from Shan Dong university hospital of traditional chinese medicine were choosed and randomly divided into two groups:the An-Chang-Yu-Yang Decoction group and the Solfasalazine(SASP)group, each of the thirty cases. then their clinic curative effects were observed after corresponding treatments. First, we observated the two groups in two months.second, we observated the relapse rate in six months. At the same time,the curative effects were checked and the curative mechanism was searched by the experiments of animal model.Results:The treatment group's synthesis effective of clinical, the effective of syndrome of TCM, relapse rate.and change of platelet count, enteroscopy and pathologic observing are better than the control group's. (P<0.05).The animal experiments showed:(1)The treatment groups had good effects of anti-inflammatory and relieve pain. And the medium dose group of modified anchangyuyang decoction is the best.(2)The level of IL-6 in rats' colon tissue in the model group was significantly higher than that in normal group;the level of IL-13 in rats' colon tissue in the model group was significantly lower than that in normal group.After treatment, the level of IL-6 in rats' colon tissue in every treatment group was significantly lower than that in the model group;the level of IL-13 in rats'colon tissue in every treatment group was significantly higher than that in model group. Among the treatment groups, the best effect was the medium dose group of modified anchangyuyang decoction (compared with the SASP group in the level of IL-6or IL-13, P＜0.01 or P＜0.05; compared with model group, P<0.01);the effect of the high dose group of modified anchangyuyang decoction was equivalent to that of the SASP group(compared with the SASP group in the level of IL-6 and IL-13, P>0.05). the low dose group of modified anchangyuyang decoction had the worst effect. (compared with model group, P>0.05.)(3)Compared with normal control, there was increased expression of NF-κB in the experimental colitis rats(P<0.01).Compared with experimental colitis, the expression of NF-kB in the medium dose group of modified anchangyuyang decoction was significantly reduced. (P<0.01), Compared with experimental colitis, the expression of NF-κB in the high dose group of modified anchangyuyang decoction and the SASP group was also reduced. (P<0.05), the low dose group of modified anchangyuyang decoction had the worst effect(compared with model group, P>0.05).Conclusion:the An-Chang-Yu-Yang Decoction is a effective drug in curing the recrudesced or persisted type ulcerative colitisin in active state, through clinical and experimental observation,The mechanism of the An -Chang-Yu-Yang Decoction effectiveness might be:(1)it can anti -inflammatory and relieve pain rapidly. (2)Modified An-Chang-Yu-Yang Decoction decoction can regulate the abnormal immune response by decreasing the level of IL-6,increasing the level of IL-13 in colonic mucosa of UC and inhibiting the expression of NF-κB.(3) it can improve the blood circulationof intestinal mucosal and promote recoverment of pathological changes.
