Objective:To investigate the distribution of the perforating branches artery of pedicle flap of sural nerve nutrient vessels, explore the feasibility of new method and its clinical application. Methods:Observed on 10 adult cadaveric's low limbs, measure the position and length of ramiperforantes blood;Summarize the 25cases of soft-tissue defects in the ankle and foot from March 2008 to august 2010.Results:The pedicle superficial sural artery island flap was used in 25 cases, all flap were survived. Anatomy of cadaver 10 cases, color Doppler ultrasonic inspection to normal gay 20 cases, the shallow arterial of sural is accompany with the neurovascular, at the point of 5cm over the lateral malleolus, Those perforating branches included fascia branches,cutaneous branches, nerve and vein nutrient branches. Those nutrient vessels formed longitudinal vessel chain of sural nerve shaft, vessel chain of vein side and vessel network of deep superficial fascia. Conclusion:By differentkind of approach can identify the rotation point of the artery at the point of 3cm over the lateral malleolus,it is good for operation,and get high success to repaire soft tissue defects in the inferior part of the leg,ankle and foot when rotation pointofflap is close to the cusp of lateralmalleolus.
