Observing: To Reveal the relationship between the different ratio of the length and width of slender narrow pedicle and random flaps survival area,and observe the changes of TGF-β1 and MDA express in the flaps survival process.Methods:①25 pigs were randomly divided into 5 groups, there were 5 pigs for each.②The ratio of length to width of slender narrow pedicle in 5 groups respectively was: 0cm:2cm1cm:2cm,III 2cm:2cm,3cm:2cm,4cm:2cm. Each ratio's slender narrow pedicle was carrying 5 different Roundly random flaps, diameter of which respectively was 2cm,3cm,4cm,5cm,6cm, and were named after A, B, C,D, and E flaps . Besides ,flaps A was controlled with flaps B,C,D,E. In each group, flaps A, B,C, D and E flaps were created on both sides of pig's back. Flaps were randomly arranged, and the pedicles were 4cm away from midline of the back, 4cm spacing between flap and flap.③Gross observation of each flap, flap survival after 14 days of observation of the situation, the standard measurement of the survival length of the grid paper, and calculate the survival area. Intraoperative, Postoperative 3 days, 5 days, 7 days, 14 days, respectively, cutting the distal lateral margin of flap of each group,the size of a long (0.5cm)×width (0.3cm) full-thickness flaps,as specimens.④By HE Staining,tissue microarray, immunohistochemistry, enzyme-linked immunosorbent (ELISA) technology, TBA method to carry out qualitative and quantitative analysis 0f TGF-β1, MDA in flaps tissue.Results:①When the proportion of the length and width of slender narrow pedicle random flaps is certain ,with the increase of the flaps area, the flaps survival area also will be increasing, but when it reached a certain limit that the distal flap necrosis occured.②When the size of the flaps remain unchanged, with the increase of the ratio of length to width of slender narrow pedicle, the flaps survival were not affected, but up to certain limits which the distal necrosis happened.③Histological examination revealed that with the ratio of the length and width the slender narrow pedicle increased, the TGF-β1 and MDA levels increase in the flaps; after 3 days, the MDA and TGF-β1 levels of flaps distal were higher than the baseline, after 5 days to reach the peak, then gradually drop content,after 7 days ,significantly decreased, after 14 days,close to the base value.Conclusion:①When the narrow pedicle of the aspect ratio will narrow, the increase in the area with the flap, flap survival area also will be increased, but reached a certain limit that occurs when the distal flap necrosis.②When the size of the flaps remain unchanged, with the narrow strip of the increase in aspect ratio pedicle flap survival is not affected, but up to certain limits which happened when the distal necrosis.③TGF-β1 plays an important role in the different ratio of the length and width of slender narrow pedicle random flaps survival process.④MDA is an important indicator in ischemic injury of the different ratio of the length and width of slender narrow pedicle random flaps.
