Objective:The research will be studied from the difference of expression of COX-2, VEGF, BMP-2 between focuses in osteosarcoma without metastasis and primary/lung metastasis of osteosarcoma with lung metastasis, to discuss the role of COX-2, VEGF, BMP-2 in process of lung metastases.Method:We collected 33 cases of osteosarcoma from June 2001 to June 2007 in Second Affiliated Hospital Zhejiang University School of Medicine Department of Orthopaedics. Among the total, 10cases with pulmonary metastasis examined by pathology;11cases with pulmonary metastasis checked by clinical examination; 12 cases followed up by more than 5 years without recurrence. The methods of two-step Envision immunohistochemistry were explored to detect the expression of COX-2,VEGF,BMP-2.Results:The positive expression of COX-2, VEGF and BMP-2 were localized in the cytoplasm. The rates of positive expression in osteosarcoma without metastasis and primary/lung focuses of osteosarcoma with lung metastasis were 25.0%(3/12),33.33% (4/12),33.33%(4/12),81.81%(9/11),81.81%(9/11),81.81%(9/11) and 70.0% (7/10),80.0%(8/10),80.0%(8/10). The rates of positive expression in primary/lung focuses of osteosarcoma with lung metastasis were significantly higher than those found in osteosarcoma without metastasis (P<0.05).There was no difference between the primary and lung focuses of osteosarcoma with lung metastasis (P>0.05) There was a strong rate of lung metastasis in patients with high positive expression rates of COX-2, VEGF,BMP-2 primary focuses. Correspondingly, The high positive expression of COX-2,VEGF,BMP-2 in lung focuses means poor prognosis.Conclusions:The rates of positive expression of COX-2,VEGF,BMP-2 in primary/lung focuses of osteosarcoma with lung metastasis were significantly higher than those found in osteosarcoma without metastasis. The positive rates expression of COX-2 was related to the expression of VEGF. The patients with positive expression of COX-2,VEGF,BMP-2 had high probability of pulmonary metastasis and with poor prognosis.
