[Object] To observe the expression of CXCR4,MMP-9,Survivin and NF-κB on adrenocortical carcinoma(ACC), adrenocortical adenoma(ACA), adrenal hyperplasia and normal adrenal cortex tissues, to investigate diagnostic indication of adrenocortical tumor. [Methods] (1) Using RT-PCR and immunohistochemistry to detect the expression of CXCR4,MMP-9,Survivin and NF-κB on adrenocortical carcinoma cell line (SW-13), injecting SW-13 cells into nude mice to establish subcutaneous tumor model, then to detect the expression of 4 protein markers mentioned earlier on subcutaneous tumor cells with RT-PCR and immunohistochemistry after the model was established. (2) Using SP immunohistochemical staining to assay the expression of the 4 protein markers on clinical specimens, including 45 cases of adrenal cortical tumors (15 cases of ACC, 30 cases of ACA), 11 cases of adrenal hyperplasia and 7 cases of normal adrenal cortex tissue. [Results] (1) After 6 weeks, SW-13 cell subcutaneous xenograft model was established. (2) the expression of CXCR4,MMP-9,Survivin and NF-κB at RNA and protein level are both positive on sw-13 cells and subcutaneous tumor, the expression of which is consistent. (3) CXCR4 positively expressed in 13/15 cases of ACC, higher than which in ACA (6/30), the difference was statistically significant (P = 0.00). In Adrenal hyperplasia and normal adrenal cortex tissue were not detected positive expression of CXCR4. The positive expression of MMP-9 in ACC is the highest (9/15), ACA followed (5/30), the expression in ACC compare with which in ACA, the difference was statistically significant (P=0.0056). MMP-9 was positively expressed in small amount of adrenocortical hyperplasia (1/11), the difference in ACC between adrenocortical hyperplasia and normal adrenal cortex is statistically significant (P=0.0099, P=0.001). The positive expression of Survivin in ACC is the highest (7/15), ACA followed (10/30), the expression of Survivin in adrenal hyperplasia is rare (2/11), no positive expression of Survivin was detected in normal adrenal cortex tissue. The difference of which in ACC between ACA,adrenal hyperplasia and normal adrenal cortex tissue is no difference(P=0.25). The positive expression of NF-кBp50 in ACC is the highest (9/15), ACA followed (8/30), adrenocortical hyperplasia is rare (2/11), the difference between ACC and ACA is statistically significant (P=0.022). The difference of which in ACC between adrenocortical hyperplasia group and normal adrenal cortex tissue is statistically significant (P=0.024, P=0.010). The positive expression of NF-кBp65 in ACC is the highest(10/15), ACA followed(18/30) , adrenal hyperplasia group is rare(4/11). Between ACC and ACA has no statistically significant difference (P=0.44). the difference is significant (P=0.04) between ACC and adrenal hyperplasia group,the difference between ACC and normal adrenal is statistically significant(P=0.0043). [Conclusion] (1) It was a convenience method for ACC researches that using SW-13 cell line to establish ACC subcutaneous tumor model. (2) The expression of CXCR4, MMP-9 and NF-кBp50 in ACC are significant higher than which in ACA, therefore, they have important value of the differential diagnosis to adrenal cortex tumor.
