Abstract/Summary:
Objective:To explore the efficiency and safety of self-prepared Roeder's tourniquet to prevent hemorrhage in laparoscopic myomectomy; to determine its clinical hemostatic value in laparoscopic myomectomy.Methods:We design self-prepared Roeder's tourniquet on the basis of the Roeder knot. to reduce the blood loss in Laparoscopic myomectomy.32 patients were treated with laparoscopic myomectomy because of uterine myoma in Department of Obstetrics and Gynecology of Qilu Hospital of Shandong university in July 2008~March 2009. They were assigned into 2 groups randomly:self-prepared Roeder's tourniquet group (study group)(15) and control group of conventional LM group(control group)(17). There were no significant differences in ages, history of pregnancy of the patients. Intra-operative control group was injected pituitrin 6IU adding sodium chloride brine 10ml in the uterus muscle before the fibroids removed. On the basis of control group, Patients in study group intra-operative were used tourniquet made of number 8 catheter and number 1 absorbable suture to stop bleeding. Observe several indexes, including the amount of bleeding, postoperative morbidity, operating time, discharge time etc. The results expressed as mean±standard deviation (χ±s), were analyzed by statistical software SPSS18, measurement data by t test, and count data byχ2 test. P<0.05 was considered statistically significant.Results:One case of control group was changed to laparotomy because of extensive dense adhesion. The remaining 31 lesions were successfully treated by surgical means without complication occurred.1. Operating timeThere were no significant differences in the mean operating time between study group (79.66±15.79min) and control group (83.75±20.84min),P>0.05.2. The amount of bleedingThe mean blood loss of the study group(83.6±14.75ml) is significantly less than the control group (176.31±86.05ml). Difference between the two groups was significant, P<0.001.3. Postoperative Observation indexesThere were no significant differences in the exhaust time, postoperative complications and average hospitalization time after operation between study group and control group.Conclusion:This study suggests self-prepared Roeder's tourniquet can significantly reduce blood loss in laparoscopic myomectomy, without extending the operation time. As to the recovery after operation and postoperative complications, there are no significant differences between the two groups.Self-prepared Roeder's tourniquet is a convenient, effective and feasible method to stop bleeding in laparoscopic myomectomy and worthy of extending in laparoscopic myomectomy.
