ObjectiveTo compare the relationship between the recrudesce of the Second Primary Malignancies and the glottic carcinoma, and to study the risks of the recrudesce of the glottic carcinoma.MethodsWe followed up patients in T1-T3 midcord from 2001 Oct to 2009 Oct . We analyze the proportion of recrudesce in the contralateral of vocal cords in the patients who had the operation over 3 years. Also, we followed up the patients who had the operation in 1-3 years by electrolaryngoscope examination, and gave a biopsy to the patients who had a potential recrudesce in the contralateral of vocal cords. For the current patients, we give the biopsy during the operation in the anterior 1/3 of the contralateral of vocal cords. We compare the causes in the patients who had the operation between the ones of recrudesce in the contralateral of vocal cords and the ones with non-recrudesce. And we also compare the causes in the current patients between the ones of recrudesce in the contralateral of vocal cords and the ones with non-recrudesce.Results21 out of 84 patients who were followed up by 3-8 years were recrudesced. 10 patients of them recrudesced in the primary region, 5 patients recrudesced in the neck And 6 patients recrudesced in the hemilaryngeal beyond the primary region, it accounts for 7.2% of the total patients, and it takes 28.6% of the patients who were recrudesced. 55 patients were followed up by 1-3 years, 6 of them were recrudesced, 1 of them were recrudesced in the contralateral, it accounts for the 1.8 of the total patients, and it takes 16.7% of the patients who were recrudesced. Atypical hyperplasia in the were detected in 12 patients. In the 30of current patients, 2 patients were detected of cancerization in the contralateral of vocal cords, 7 of them were suffered atypical hyperplasia, and 21 of them with no pathological changes.ConclusionsSecond Primary Malignancies is a significant cause for the recrudesced of glottic carcinoma. There about 30% of the total patients were recrudesced.And with the period after the operation, the risk to suffer the recrudesce is increased. Smoking and esophageal reflux are also the risks of the recrudesce in the contralateral of vocal cords, and they are the important causes for the pathological changes in the contralateral of vocal cords as well.
