Objective:To investigate incidence, cause and mortality of acute respiratory distress syndrom(ARDS) in intensive care unit in Jincheng mineral general hospital.Methods:Patients in ICU of Jincheng mineral general hospital from 2004 to 2009 were retrospectively studied. ARDS was diagnosed according to the 1994 American-European consensus conference criteria.Rusults:Of 1714 patients admitted to ICUs in the same period,there were 60 patients (3.5%) diagnosed as having ARDS. In 16 pulmonary ARDS cases (26.7%),10 died, mortality was 62.5%. In 44 extro-Pulmonary ARDS patients (73.3%),21 died, mortality was 47.1%. Pulmonary ARDS mortality was significantly higher than the extro-Pulmonary ARDS (x2=13.261, P<0.01).Major primary diseases for ARDS were sepsis (21.7%), pneumonia (16.7%),the others were multiple severe truma, surgical operation and acute pancreatitis.60 cases of ARDS patients,31 died, including Pulmonary ARDS,10 died (32.3%), and the extro-pulmonary ARDS 21 died. The overall mortality of ARDS was 51.67%. septic shock(51.61%) was major lethal cause. only 29.03% died of respiratory failure. In six years, mortality had no significant changes.Conclusion:1.In 6 years, the average incidence of ARDS was 3.5%.the lowest was in 2007.Comparing 2004 with 2009,the incidence was slowly rising. From 2007 until 2009, Incidence was increasing annually 2. the mortality remains high(51.67%), Pulmonary ARDS mortality was significantly higher than the extro-Pulmonary ARDS,and there is no tendency to decline in recent years.3.Critical prtients was not died of ARDS simply. Because of the progress of the ventilation,ARDS mostly died of septic shock.4.when septic shock or MOF accured, the mortality was increased distinctly.
