ObjectiveTo investigate the clinical features of invasive pulmonary aspergillosis (IPA) enhance the awareness of the disease, to evaluate the plasma (1-3)-B-D-glucan detection of clinical significance.MethodsWe reviewed the underlying diseases, symptoms, signs, radiology manifestations and treatment and of1 3 IPA patients admitted to our hospital from July 9,2007 to December 28,2009.And collected 15 probable IPA patient to be admitted the same period,collected 14 cases of healthy volunteers formed the control group to detect the plasma (1-3)-B-D glucan levels between three groups of data with or without statistical significance.Results13 cases of IPA patients, underlying diseases combined,12 cases, of which 7 cases of tuberculosis (53.8%), COPD 3 patients (23.1%), bronchial asthma,4 cases (30.7%), lung cancer after chemotherapy in 1 case (7.7%), blood system diseases in 3 cases (23.1%), organ transplant recipients and 1 (7.7%); no underlying disease in 1 case. The main symptoms are cough, sputum in 9 cases (69.2%), fever in 7 cases (53.8%), hemoptysis in 3 cases (23.1%), chest pain,4 cases (30.7%), dyspnea in 3 cases (23.1%). Imaging features of 13 cases were performed after admission in patients with lung CT examination showed an empty shadow (5 cases), bronchial pneumonia-like manifestations (3 cases), empty nodules inside-the air a half syndrome (2 cases), diffuse nature of Small nodules (2 cases), group massive shadow (1 case). G laboratory tests, sputum culture-positive treatment effect all the 13 cases of patients were treated with antifungal therapy (voriconazole, itraconazole, or caspofungin) improved in 10 cases,1 case the termination of treatment because of adverse reactions,1 case of economic the reasons for withdrawal of treatment,1 patient died due to underlying diseases, with a total effective rate 76.9%Statistical Analysis (1-3)-β-D-glucan levels of comparison:clinical diagnosis group compared with the control group significantly (t= 4.095 p<0.05), clinical diagnosis group and patient group differences to-be statistically significance (t= 3.539 p<0.05), diagnosed group and the normal control group, the difference was statistically significant (t= 5.578 p<0.05)ConclusionInvasive pulmonary aspergillosis over the clinical manifestations included cough expectoration, hemoptysis, chest pain; imaging complex and diverse, halo sign, air crescent sign is a typical manifestation of this disease prompted the diagnosis has an important role; when suspected IPA plasma (1-3)-β-D glucan detection, increased the level of significance of important tips;Early treatment can achieve significant effect.
