ObjectiveTo observe the histopathologic and morphological changes of palatopharyngeal soft tissues in patients with obstructive sleep apnea hypopnea syndrome(OSAHS) patients.Materials and Methods38 OSAHS patients,all of them were male,from 2009-3 to 2009-11 in the otorhinolaryngology department of the first affiliated hospital of CMU.Diagnosed by polysomnography(PSG,AHI>5 could be diagnosed),dividing the patients into 3 groups according to AHI,they are mild group (540,n=15),all acceptted uvulopalatopharyngoplasty(UPPP). The sections were stained by HE.Observe the histological quantitative changes of the specimens in and analyze the glandular tissue,vascular components and interstitial elements.And make Statistical analysis.Results1,Optical microscope deserved(100 times)(1) Epithelial mucosa of soft palate of mild OSAHS group are stratified squamous epithelium,have regular shape, cell layered neatly arranged, submucosa are loose tissue and thickening of the lamina propria, a small amount of fat cells exist; the soft palate squamous epithelial cells of moderate group are swelling and have keratosis, basal cell degeneration of some liquid, mucous membrane and submucosal connective tissue containing a small number of lymphocyte infiltration; the soft palate squamous epithelial cells of severe group soft are swo(?)en,irregular and hyperkeratosis, mucosa and submucosal layer of loose connective tissue containing inflammatory cells, and shows a lot of fat vacuoles.(2)The soft palate mucous acini of mild OSAHS group are consistent, clustered distribution, almost all mucous acini have the clearest cytoplasm;the soft palate mucous acinar of moderate group have slightly inconsistent and irregular shape, between acinar cell could see a small amount of fat visible infiltration, no mucous acinar serous acini have metaplasia;the soft palate mucous acini of severe group are apparent inconsistency,have apparent irregular shape, between acinar cell show visible fat infiltration, some mucous acini are replaced by deeply stained cytoplasm serous acinar.(3) Palatoglossal muscle and palatopharyngeus muscle fibers of mild OSAHS group were arranged regularly, the structure are clear and have regular shape; muscle fiber of moderate group are clearly visible, less structured forms, arranged irregularly, we can see a little fat cell infiltration; muscle fiber of severe group can be identified, structure are disordered unclearly, we can see a lot of fat infiltration, muscle fibers were pleomorphic changes, including swelling, atrophy and degeneration, some of elastic fiber disruption.2,The constituent ratio of soft palate of three kinds of ingredientsChanges of glandular tissue and interstitial elements between mild, moderate and severe OSAHS group patients, statistically significant differences(P< 0.05). Vascular components between mild and severe group,moderate and severe group,the changes have statistically significant differences(P< 0.05);the changes between mild and moderate group,have no statistically significant differences(P>0.05).Conclusionwith the rising of severity of OSAHS patients,there are a lot of histopathologic and morphological changes of palatopharyngeal soft tissues in patients.
