PerfaceWrong in the Neurology and Neurosurgery disease complexity and diversity. Ischemic disease of which occupy a certain proportion of them, cerebral ischemia is a common neurological disease, the pathological process inside and outside, so for the prevention and treatment of cerebral ischemia has become today's hot spots. Kitagawa et al found that short-term non-fatal brain hemorrhage, increase of the lethal brain ischemic tolerance, and made ischemic preconditioning (neuronal ischemic preconditioning, NIPC) concept. NIPC's protection mechanisms when in fact the body in the release of a small amount of transient ischemic injury factors, start endogenous protection mechanism for neurons, tissue ischemia tolerance.Inflammatory factors in cerebral ischemia injury played an important role in inflammation as the inflammatory chain reaction "gateway" TLR4 protein involved in this process. This issue primarily through the establishment of ischemic preconditioning and cerebral ischemia model to observed changes in TLR4 expression, then further cerebral ischemic preconditioning and TLR4 expression.Material and MethodsAdult SD rats were randomly divided into three groups:Ⅰcerebral ischemic preconditioning and ischemia group, monofilament method in the middle cerebral artery occlusion, ischemic preconditioning 3min, reperfusion 72h, and then ischemia 6h;Ⅱbrain ischemia group, middle cerebral artery occlusion caused by cerebral ischemia and 4h;Ⅲsham operation group, only the line is not inserted into the carotid artery 4h. The rats on the functional score, select the test object can be selected, to be executed over anesthesia. Brain tissue conditions, reject the group. The small numbers of pathological specimens were obtained to be stained with water necrosis. It issues (ELISA) determination of TLR4 and IL-6 levels. SPSS 13.0 statistical software used for data processing, measurement data were presented as mean±standard deviation (x±s) that the groups were analyzed using 22 compared with a multiple comparison t test.ResultThe whole rat cerebral ischemic preconditioning groupⅠandⅡⅢcerebral ischemia group compared with sham group had cerebral edema, but edema area in groupⅠand groupⅡlevel is less than that. HE stain on pathological cerebral ischemic preconditioning groupⅠandⅡⅢcerebral ischemia group compared with the sham operation group were seen compared to inflammatory cell infiltration, edema and necrosis of nerve cells. However, groupⅠedema, inflammatory cell infiltration and necrosis is less than that in groupⅡ. ELISA determination of TLR4 and IL-6 levels, among all groups by the statistical test were significantly different,ⅠandⅡischemic preconditioning group compared with cerebral ischemia group, the former TLR4 and IL-6 levels than The latter lower (P<0.05),Ⅲ,Ⅰsham group and ischemic preconditioning group, the former of TLR4 and the level of interleukin-6 lower than the latter (P<0.05),Ⅲsham group andⅡcerebral ischemia group, the former of TLR4 and IL-6 levels lower than the latter (P <0.05). Among the groups were significantly different.ConclusionAfter brain short slight ischemia pretreatment (3min), again when vice minister the diencephalon ischemia (4h), its inflammation responded the comparatively sole ischemia pretreatment is long when the diencephalon ischemia lightly, the TLR4 expression decreases, the inflammatory factor IL-6 content decreases. The showing ischemia pretreatment has the protection function to the brain ischemia, the TLR4 expression and the brain ischemia order of severity is related in the brain ischemia, the brain ischemia pretreatment may reduce TLR4 to express, reduces the inflammation response.
