Impact Of HLA Matching On HCMV Viremia After Liver Transplantation For HBV-cirrhosis

Aim In this paper, we investigated whether human leucocyte antigen (HLA) matching influence the incidence of HCMV viremia, the time point of HCMV recurrence, the incidence of graft failure and the time point of graft failure after diagnosis of HCMV viremia after liver transplantation (LT) for HBV cirrhosis.Methods In a retrospective analysis we were reviewed 91 recipients of LT for HBV cirrhosis and complete HLA-typing for outcome after LT. All the patients were divided into different groups according to the HLA-A,-B, and DR locus matching. Meanwhile, we will receive some clinical data in these patients, such as the incidence of HCMV viremia, the time point of HCMV occurrence, the incidence of graft failure, and the time point of graft failure after diagnosis of HCMV viremia. HLA typing was performed using a sequence-specific primer-polymerase chain reaction (SSP-PCR) kit. The patients were examined serologically for hepatitis B and HCMV using a commercial kit.Results The incidence of HCMV viremia post-LT was 81.32%. The overall 4 year figure of graft failure was 16 of 91 (17.58%) patients after diagnosis of HCMV viremia. The incidence of HCMV viremia was 100% in two HLA-A locus compatibility. It was 91.43%(32/35) in only one HLA-A matching group and was 72.55%(37/51) without HLA-A compatibility (p=0.027). The degree of HLA-B or-DR matching, however, was not influence the outcome. The time point of HCMV viremia, graft failure, and the time point of graft failure after diagnosis of HCMV viremia were independent on the HLA compatibility (p>0.05).Conclusion An interesting discovers was found that the risk of incidence of HCMV viremia tended to be higher in patients with HLA-A better compatibility. Graft failure, time point of HCMV viremia and graft failure after diagnosis of HCMV viremia, however, may be independent on HLA allele compatibility.
