Objective: To construct a new saccular aneurysm model in rabbit's carotid arteries with external jugular vein segments by Microsurgery. Using computer simulation software simulate the incomplete clipping construction of aneurysm model, Contrast and analysis the change of hemodynamic in the aneurysm model.Methods: Under general anesthesia snipped a segment about 3~5mm long from the Rabbit's left external jugular vein, Then transplant the vein segment to the left carotid arteries by Microsurgery, Constructed carotid arteries sidewall aneurysm model. Two weeks later, Using TCD check the Rabbit's left carotid arteries and aneurysm, collecting flow velocity data in left carotid arteries and aneurysm. Then observation the construction of carotid arteries and aneurysm for select successful experimental animals through intra~ear venous digital subtraction angiography(DSA), then was intra~arterial digital subtraction angiography to left left carotid arteries, measure the size of left carotid arteries and aneurysm. After above~mentioned step, implement 3D~DSA to aneurysm model and three~dimensional reconstruct data. The aneurysm animation data were transformed in computer, new format is AVI. Simulate the data by computer simulation software(CFD). Using CAD drawing the image of aneurysm, drawing the original image and after incomplete clipping image. The data was substituted into the formula of FLUENT, divide reseau to build computer data model. Set rabbit's cardiac cycle is 0.3S, set average flow velocity and maximum flow velocity into FLUENT. Obtain maximum shear force and flow turbulence in the aneurysm. Contrast data and take statistical analysis.Result: All of test rabbits are alive. Saccular aneurysms formed in 11 of 13 rabbits in test group. 2 rabbit were failed to form the aneurysm model. The width diameter was in range of 4.11~5.51mm, the height diameter was in range of 4.02~5.57mm, the aneurysm neck diameter was in range of 2.02~3.20mm. Contrast before~operated and after~operated the most shear force, taking statistical analysis, we concluded: there are significant difference(P<0.01).Conclution: 1. The method of transplant vein segment to arteries construct aneurysm model is simple, reliable, controllability and repeatability. we can establish stable saccular aneurysms in the rabbits by this way. Animal is high survival~rate, short experimental period. 2. The shear fore and turbulence in aneurysm incomplete clipped are significant difference with original aneurysm . We should inverse direction of blood flow occlusion the aneurysm in operation. In the situation, if we could not complete clip the aneurysm or folder off from aneurysm, the folder can reinforcement the maximum forced place on aneurysm neck, prevent aneurysm continue to grow.
