Silicosis is the common occupational disease which threat seriously to people's health. The examination and diagnosis of silicosis in the past rely mainly on conventional X-ray, there is a certain degree of misjudgment. In recent years, with the rapid development of computer technology, especially high-resolution CT (HRCT) of the clinical application, the diagnostic accuracy of silicosis began to be greatly enhanced. Although there are still some issues that must be resolved, the CT (HRCT) examination in the diagnosis of silicosis has been concerned and considered to have greater potential and application prospects. In this paper, the HRCT and X-ray images of silicosis from 80 cases were compared and analysis, in order to explore their value in PhaseⅡdiagnosis of silicosis. The data from 80 cases of silicosis were collected in Shenyang City 9th People's Hospital during 2006.1-2009.9. Every patient has not only a clear history of occupational diseases, but also two or more image of X-ray and HRCT which was suit for the diagnostic requirements. According to diagnostic standard (GBZ70-2002) of silicosis by X-ray, the patients were diagnosed with stageⅡsilicosis. The data obtained was differential count. the paired Chi-square analysis was used to compare among the groups. P<0.05 was considered statistically significant.The results are as follows:⑴there are 10 images index with differences in the number of detected cases between the X-ray and HRCT imaging results. Except for "pale zone", the cases from HRCT were more than ones from X-ray. Except for holes, spot and slice, small shadows gathered, there are seven indicators with significantly difference (P <0.05) by the paired chi-square test.⑵The number of lung-zones where the various grade with the small shadow-intensive distribute. are quite different The number of lung-zones with 0-1 grade from HRCT is much more than ones from X-ray. The number of lung-zones with 2-3 grade from HRCT is much less than ones from X-ray, and there is a significant difference (P <0.05).⑶The distributing range of the small shadow from HRCT is larger than one from of X-ray, and Abstract impact silicosis diagnosis by X-ray. In this paper, 8 patients those were diagnosed silicosis stageⅡby X-ray were diagnosedⅡ+ by HRCT. 13 patients those were diagnosed silicosis stageⅡ+ by X-ray were diagnosedⅢby HRCT. By paired chi-square test, there was a significant difference (P <0.05) in staging of silicosis between X-ray and HRCT.⑷compared with the corresponding HRCT image about 3 grade of the small shadow-intensive, X-ray film showed many small shadow and pulmonary grain disappeared, while the image from corresponding HRCT mainly showed irregular coarse reticulated texture and accompanied by some small shadow.⑸there are obvious difference on the diagnosis of silicosis complications. In this paper, the quantity of the five kinds of silicosis complications detected by HRCT were higher than ones detected by X-ray. The cases of silicosis complicated with lung cancer detected by HRCT is the 3 times to ones detected by X-ray, and there is a significant difference (P <0.05).Based on the image analysis, conclusion is shown as following:⑴the result of accurate classification and micro-nodules of Silicosis detected by HRCT is superior one by X-ray.⑵The degree silicosis lesions shown by HRCT is usually higher than one by X-ray.⑶There are still overlapping and low-resolution image in high-kV X-ray film, which is difficult to differential diagnosis.⑷The determine of small shadow-intensive by HRCT is less than one by conventional X-ray.⑸There is a high complementarity in the silicosis diagnosis of phaseⅡbetween X-ray and HRCT...
