| Objective: The purpose of the study was to assess the effect of various hypertension on arteriolar and venular diameter, by using a computer-assisted measurement to measure the fundus vessel calibers from the simple diabetic patients and the hypertensive diabetes. To evaluate the effect of hypertension on the retinal blood flow within DR objectively, reflecting the status of the retinal blood supply, so as to guide the treatment of improving the retinal blood supply and the therapeutic effect among the various types of DR patients. Methods: Selected the fundus fluorescence angiography images of our hospital partners in diabetic patients with or without hypertension. 96 cases were selected from patients of 51~60 years old, among which 48 are female and 48 are male. These cases were grouped in terms of DM, DM with grade 1 EH,DM with grade 2 EH, DM with grade 3 EH. A grader identified all retinal vessels located 0.5~1.0 disc diameter from the optic disc margin, and used ImageJ program to measure the width of these vessels. The temporal artery and it's corresponding vein within right eye selected were less variation. To use the mouse to point both sides of the blood vessels, being vertical to the vessel edges, the results was showed automaticly and each section was measured five times for average. These images were converted into 1320 x 1032pixels, 8-bit, 130 MB, storage formats for JPG format. After the measurement was completed, the datas were gathered for statistical analysis. Results: Using SAS to analyze these datas. Compared to the cases of 0 EH-DM, measurements of RAD did not show significant change for the cases of level 1 EH-DM, but become narrower for the cases of level 2 and level 3 EH-DM. Compared to the cases of 0 EH-DM,measurements of RVD did not change significantly among the groups. Compared to the cases of 0 EH-DM, AVR measurements did not show significant change for the level 1 EH-DM, and the ratio grew smaller for the cases of the level 2 and level 3 EH-DM.RAD,RVD and AVR measurements did not show significant variation between level 2 and 3 EH-DM,the similar phenomenon to the measurements between 0 EH-DM and level 1 EH-DM.Conclusion: Level 2 and above hypertension had a greater effect on RAD narrowing. Hypertention had no significant effect on RVD. RAD narrowing was shown more sensitive than AVR decreasing in this study. This study was carried out using 96 cases of 4 categories,a relatively small set of samples. For a more complete study,a large set of samples should be collected and used in futural research. |
