Objective To evaluate the possibility and accuracy of real-time three-dimensional echocardiography(RT-3DE) in the quantitate assessment of the global and regional right ventricular systolic function in atrial septal defect(ASD) patients with different pulmonary artery systolic pressure(PASP), and to explore the simple noninvasion way to evaluate the right ventricular systolic function for clinical operation.Methods RT-3DE was performed in one hundred and thirty-one ASD patients,which had been divided to four groups according their PASP: ASD non pulmonary artery hypertension(PAH),ASD with mild PAH,ASD with moderate PAH and ASD with severe PAH.We had measured the global right ventricular systolic function(RVEF),and regional right vectricular systolic function(rEF) which was including inflow tract component,outflow tract component and apical component,and compared all of them with thirty-four normal controls'.Through the result of comparing,we had evaluated the possibility and accuracy of measuring the global and regional rightventricular systolic function in patients with different PASP by RT-3DE,and debated the tendency of the right ventricular systolic function in different degrees of PASP.Result The RVEF and the rEF appeared to reduce gradually in pace with the increasing of the PASP approximately in the ASD patients,and which of ASD patients with moderate and evere PAH were lower significantly than those obtained from other three groups;outflowEF could use to assess the RVEF as a credible index.Conclusion Global and regional right ventricular systolic function in atrial septal defect(ASD) patients could be assessed quantitive,fast,and with no invasive by RT-3DE and Qlab analysis software, and the influence of different pulmonary artery pressure on global and regional right ventricular systolic function could also be detected early. RT-3DE can provid evaluation proof of ASD patients'right ventricular condition for clinic.
