Objective: To assess the application value of Recombinant Human Ad-p53 Injection (Gendicine) hepatic arterial chemoembolization for the treatment of advanced primary hepatocellular carcinoma.Method: The diagnosis of advanced primary hepatocellular carcinoma was made by hepatoncus pathological biopsy and immunohistochemical detection, 40 cases with positive expression of p53 were divided into two group : control group (n=20) and experimental group (n=20).The patients in experimental group were treated with conventional TACE in combination with two doses Gendicine that were added into Iodinated Oil; routine TACE was used in the control group. Three TACE should be done on both group. With taking the third TACE as standard, we assessed the postoperative clinical therapeutic effect of two group .Result: The effective rate of experimental group and control group were 95% , 70% respectively. The tumor body was reduced more significantly (P=0.0428<0.05), AFP level was decreased more remarkably(P=0.0261<0.05), Karnofsky score was higher(P=0.0031<0.05), the postoperative fever and chill was more common than other adverse effects(P<0.05), there was no significant difference among the other adverse effects(P>0.05).compared with control group, the occurrence of tumor body enlargement after treatment in the experimental group was prolonged(P=0.029<0.05). There is no significant difference in the twelve-month survival rate and the time of metastasis between two group(P>0.05).Conclusion: The treatment of in combination with TACE for patients with advanced primary hepatocellular carcinoma is characterized by high local concentration in tumor tissue. The effect of drug itself can last much longer and is not easily lost with blood circulation. Gendicine is capable of bringing about a synergistic effect and strenghthening the anti-cancer effect in combination with chemotherapeutical drugs. It can improve the life quality of patients and prolong the occurrence of tumor body enlargement after treatment. The curative effect of Gendicine in combination with TACE is superior to TACE alone. The method with Gendicine for the treatment of primary hepatocellular carcinoma is safe, effective and practical.
