Background:It has been more than three hundred years for people to study the lymph system since Gaspaer Aselli discovered and described the lymphatic. As a component of body fluids circulation, lymph system has been confirmed extensively. It plays an important role in maintaining the balance of body fluids. However, lymph fluid is achromaticity and transparent, its wall is thin, tiny and fragile, it is in a collapse state at normal, and is hardly observed in a living body, so the research of lymph has been restrict.The traditionary study methods are injection and lymphangiography and so on. The commonly used dyes of chromogenic method is 3%-11% Patent Blue V (PB), which has a insignificant toxity, and absorption quickly, safely and reliablely. It can be used to demonstrate the shape and construction of the Lymphatic from macro and micro level, it provide effective morphology record for the clinical and basic research. The research of lymph system has a rapid development as the fast progress of molecular biology and immune histochemistry. It is one of the great problems about the cure of hydrops and return of lymph, which puzzles medical science still now. It has be found that the change of pressure gradient in intracelial is a prime motive for lymph return, and it can raise delivery head, promote lymphatic return. This method is the best to cure the disturb of lymphatic return. Oral negative pressure suction is just use this principle to raise the lymph recircle, and has a notable therapeutic effect on heart and cerebral vessels with insufficiency of blood and angioneurotic headache. When we use oral negative pressure to cure diseases which has related above, we found that it can improve some people intraocular pressure, so we have a doubt whether there is a relation between the lymph system and glaucoma.Glaucoma is one of the reasons that can induce eye blind in the world, and a high intraocular pressure is the chief factor. Decrease the pressure is the most important and direct method to cure glaucoma and protect visual function.. In most of the glaucoma patients, the high pressure for aqueous humor is the reason to reduce high intraocular pressure, so the power and force of lymph circle is an important problem in the research of glaucoma. It is known that there are two way for aqueous humor outflow, one is trabecular network channels, and the other is choroid-sclera channels that is also called an unclassical way. Ever since 19 century, some scholars began more and more interesting about lymph, but as the limited of method and recognition, there is no sufficient evidence for the research of lymph to make a agreement. These studies only make a vague recognition for the study of the lymph of eyes, such as there is no lymph in eyes, or has a simple relate. Although lymph system plays an important role in maintaining eyes in a normal state, the structure of the lymphatic is tiny, and lymph fluid is similar with the tissue fluid in colour, it is too difficult for us to observe and distinguish, so the research of the lymph is little or even no existence. It is commonly accepted that the lymph of eyes existent in conjunctiva and tear gland tissues. Till now, there is no proof to confirm the existence of lymphatic, but some experiments prompt that there is lymph draining between anterior/posterior of eyeballs and cervical lymph node. So we design this experiment, use anatomy method, inject PB to camera oculi anterior of rabbits, observe the chromogenic of cervical lymph node and the change of the intraocular pressure, explore whether there is draining relation between anterior chamber and cervical lymph node, and observe the effect of oral negative pressure to intraocular pressure and to the draining of anterior chamber and cervical lymph node. We expect to confirm that there is a draining relation between anterior chamber and cervical lymph node, and provide an evidence to explore new methods to treat glaucoma.ObjectiveUsing anatomical method to explore whether there is a relation between anterior chamber and the draining lymph of the neck, and to observe the effect of negative pressure in pharyngo-oral cavity on this relation and on the intraocular pressure.Methods26 New-Zealand rabbits were divided randomly into 2 groups:group A (13 rabbits), the control group (CG of LJT) without negative pressure in pharyngo-oral cavity and group B (13 rabbits), the experimental group (EG of LJT) treated by negative pressure in pharyngo-oral cavity. After anesthesia, cut them along the left jugular vein, then separate the subcutaneous tissue to the left venous angle to expose the lymphatic trucks and lymph nodes of the neck. The oral suction device are fixed. Puncture the anterior chamber and draw 0.2ml aqueous humor under aseptic. Then inject 0.2ml 10%PB into the anterior chamber, and count the time after injection. Inject the dyes to the rabbits. Group B were treated with oral negative pressure at 0.12mpa/min for 15 minutes, and 14 days as a course of treatment while group A received no treatment. Record the staining time-the anterior chamber-lymph colouration time:when the lymph nodes have been dyed after dyes injection. Observed the changes of the conjunctiva, cornea, anterior chamber and lens in anterior segment with silt lamp everyday at the same time, by the same person and use the same Schiotz indentation tonometer to measure the intraocular pressure. Compare the difference between the experimental groups and the control group.Results:1. Anterior segment changes:one day after surgery, the PB are metabolized in both groups. The cornea and lens are all clear. There is no inflammation in conjunctiva and anterior chamber in both groups. There is no statistically significance between the two groups and no statistically significance before or after the surgery in each group.2. Intraocular pressure:Before surgery,the range of pressure of group A is 10~18.8 mmHg and the average is 16.0±1.8 mmHg. After the surgery,the pressure is 10~18 mmHg and the average is 14.5±2.0 mmHg. There is no statistically significance after the surgery (P>0.05). Before the surgery,the range of pressure of group B is 12~20mmHg, mmHg and the average is 16.1±2.1 mmHg, After the surgery,the pressure is 8~13mmHg and the average is 10.6±2.5 mmHg. The preoperative and postoperative intraocular pressure is statistically significant in group B(P<0.05). The intraocular pressure of group B is significantly different compared with group A (P<0.05). The anterior chamber-lymph colouration time:Group A is (34±2.6) min and group B is (29±1.8) min. It is statistically significant between the two groups (P<0.05)Conclusions:1. It can be initially confirmed that there is a draining relation between the anterior chamber of the eye and the lymph of the neck in rabbits.2. Setup an oral negative pressure can promote the lymph circulation of the rabbits, accelerate outflow rate of aqueous humor, and reduce intraocular pressure.3. PB has no toxicity to intra ocular tissues and no effect on intraocular pressure. It can be used in the lymph research of the eye safely and reliably.4. It can be initially confirmed that the draining of the anterior chamber lymph is not through the conjunctival lymphatic vessels.
