Objective:To study the effect of pregnancy correlation factors on stress urinary incontinence (SUI) in primigravidas, and the mobility of urethrovesical junction (UVJ-M) at different periods of pregnancy and different delivery ways.Methods:Questionnaire survey was conducted among 182 primigravidas(from September 2008 to December 2008),78 SUI patients and 104 non-SUI patients,by the International Consultation on Incontinence Questionaire Short Form (ICI-Q-SF) and the UVJ-M of studied patients was measured by sonographic at 6~10 gestational weeks, 24-28 gestational weeks,36-40 gestational weeks and 6~8 weeks after delivery, respectively. The diagnostic standards of SUI:the complaint of involuntary leakage of urine when one coughs, sneezes, moves or exercises. The incidence of SUI at different delivery ways and the variations in UVJ-M between pregnancy and postpartum were analyzed by PEMS3.1 software, the relationship of correlative factors with SUI, the change of UVJ-M betweenSUI and non-SUI patients in the same pregnancy period and the change of UVJ-M under different labor models were assessed by multivariate non-conditioned logistic regression analysis, respectively.Result:1. General data:The present study included 182 primigravidas (78 SUI patients and 104 non-SUI patients) with a mean age of 25.21y±2.71 y, fetal weight of 3421.31g±451.03 g, gestation times of 1.65t±0.90t, BMI of 21.72±3.15 andΔBMI of 7.10±2.15,respectively. No significant differences were found between the two groups (SUI group and non-SUI group).2. The incidence of SUI and the variations of UVJ-M during pregnancy:2.1 the incidence of SUI:The total incidence of SUI during pregnancy was 42.86%(78/182), and the detailed SUI incidence at 6-10 gestational weeks,24-28 gestational weeks, 36-40 gestational weeks and 6-8weeks after delivery was 3.85%,13.19%,25.82%, and2.20%, respectively. The difference is significant between 6~10 gestational weeks and 24-28 gestational weeks,24-28 gestational weeks and 36~40 gestational weeks, 36-40 gestational weeks and 6-10 gestational weeks, respectively(p<0.05).2.2 the variations of UVJ-M during pregnancy:The UVJ-M at 6-10 gestational weeks, 24~28 gestational weeks,36-40 gestational weeks and 6-8weeks after delivery was 3.26mm±1.62mm,4.67mm±1.47mm,6.65mm±1.12mm and 2.64mm±0.97mm, respectively. Significantly increased UVJ-M was observed as gestational period prolonged and the difference is significant between 6~10 gestational weeks and 24~28 gestational weeks,24~28 gestational weeks and 36~40 gestational weeks, 36-40 gestational weeks and 6-10 gestational weeks, respectively (p<0.05).2.3 the comparison of UVJ-M between SUI and non-SUI:the UVJ-M of SUI and non-SUI patients at 6~10 gestational weeks,24-28 gestational weeks,36-40 gestational weeks were 4.97mm±1.96mm and 3.13mm±1.53mm,6.23mm±1.29mm and 4.42mm±1.31mm,7.35mm±0.92mm and 6.40mm±1.03mm, respectively, the difference of UVJ-M between SUI patients and non-SUI patients in the same period of pregnancy was significant (p<0.05)。3. Pre-pregnancy BMI,ΔBMI, fetal weight, striae occurring, edema, blood pressure and glucose in pregnancy, anemia, placental-position, placental weight, amniotic fluid volume and gravidity had no correlation with the SUI incidence.4. The SUI incidence and variation of UVJ-M with different delivery ways:4.1 The SUI incidence of normal vaginal delivery and caesarean section after trial of labor were 2.90%(2/69) and 2.0%(1/50), respectively. None SUI was occurredin patients undergoing vacuum extraction, and the SUI incidence was 1.67%(1/60) in selective caesarean section patients, no significant difference was observed (P> 0.05).4.2 The UVJ-M of normal vaginal delivery (69 cases) and selective caesarean section (60 cases) were 0.29mm±0.12mm and 0.20mm±0.02mm, respectively, there was no significant difference(P> 0.05). Conclusions:1. The incidence of SUI increases with the gestational weeks.2. UVJ-M goes up with the gestational weeks.3. There is no correlation between postpartum SUI incidence and delivery ways.
