Object To analyse the surgical operation of removing all kinds of insular gliomas, and the methods of decreasing the postoperative complication, in order to improve patients quality of life and prolong patients' postoperative survival.Methods We retrospectively analyzed the surgical result and prognosis of 40 patients who came from Qilu hospital from 2003 to 2009 with insular gliomas. All these patients were performed resection of insular gliomas by trans-sylvian fissure approach.In these 40 patients,21 of them are male,and 19 are female. The average age of them were 40.3, and the average disease-process were 3 month. The diameter of tumor were between 3cm to 8cm.Rsesult In 40 cases,26 had total resection, and 14 had sub-total resection,with none surgery death.The rate of total resection was 65%.5 cases had complication of transient aphasia after tumor resection, and 6 cases had contralateral limb paralysis, (among this 6cases,3 cases with completely recovery,2 cases with significant improvement, and 1 no change),3 cases had hematoma,2 cases had hematoma removing,who have permanent deficit after surgery,and 1 case recovered completely.With 1-24 months follow up,2 cases were recurrence, and 2 cases were deadConclusion Insular gliomas resection by trans-sylvian fissure approach with no or less complication is possible. Despite the challenge, removing the gliomas completely is necessary for patient to prolong their postoperative survival and improve quality of life.
